Molluscum contagiosum in HIV-lnfected Patients

Abstract: The clinical presentation of molluscum contagiosum is described in detail in 16 HIV-infected patients. Atypical facial lesions with either multiple small papules or giant nodular tumors prevailed in patients with AIDS or AIDS-related complex. A theoretical basis for the atypical localization of the disease is lacking.

“…It is not known whether the infection of HIV-positive individuals with MC represents de novo infection, acquired by bodily contact, or the reactivation of a latent condition [42]. MC is capable of inducing the formation of a variety of circulating antibodies [53], but the paucity of B-lymphocytes and of neutrophils and the tendency of recurrence suggest that humoral immunity plays but a minor role in the local immune response.…”

“…These eye-associated manifestations are most frequently encountered in young children [34], the usual sites of outbreak in immunocompetent adults being the external genitalia, the lower abdomen and the upper thigh [42]. The infection is spread by direct contact with infected individuals in showers and swimming pools [59] and on sexual contact [18].…”

“…Untreated papules in healthy people usually disappear spontaneously within several months, but in AIDS patients dense crops can persist, disfiguring infected patients. In HIV-infected people, rates of infection may be as high as 5 to 18% and as many as 33% of AIDS patients with CD4ϩ counts of less that 100 cells/mm 3 may be infected (Petersen and Gerstoft, 1992;Porter et al, 1992;Schwartz and Myskowski, 1992;Gottlieb and Myskowski, 1994).…”
