PurposeMonoclonal gammopathy of undetermined significance (MGUS) is a premalignancy preceding multiple myeloma (MM) or related disorders. In MGUS, renal impairment caused by deposition of the monoclonal immunoglobulins or free light-chains monoclonal gammopathy of renal significance (MGRS) is often associated with high morbidity and mortality. We analysed the prevalence of renal impairment, clinical features and the long-term outcome in 2935 patients with MGUS.MethodsBetween 1/2000 and 8/2016, 2935 adult patients with MGUS were identified in our database.ResultsIn 44/2935 (1.5%) patients MGRS was diagnosed. In MGRS patients, significantly more progressions to MM were observed than in MGUS patients (18% vs. 3%; P<0.001). MGRS patients showed a higher risk for progression (HR 3.3 [1.5-7.4]) in the Cox model. Median time to progression was 23 years for MGUS and 18.8 years for MGRS patients. Corresponding progression rate was 8.8 [7.2-10.7] per 1000 patient-years (py) for MGUS patients and 30.6 [15.3–61] for the MGRS group. Risk for progression within the first year after diagnosis was 1% [0.6-1.4] in the MGUS group and 10% [4-29] among MGRS patients.ConclusionThe significantly higher risk for progression to MM means MGRS patients should be monitored carefully and treated in a specialized centre.

Abnormal proliferation of plasma cells and some monoclonal B cells frequently cause the secretion of monoclonal immunoglobulins or immunoglobulin fragments into the serum, causing monoclonal gammopathy, which leads to various diseases including renal diseases. Therefore, monoclonal gammopathy is frequently associated with kidney diseases, including glomerular and tubulointerstitial diseases. Glomerular disease, with the deposition of monoclonal immunoglobulins or their components, includes monoclonal immunoglobulin deposition disease (MIDD), AL or AH amyloidosis, type I cryoglobulinemia, proliferative glomerulonephritis with monoclonal IgG deposits (PGNMID), immunotactoid glomerulopathy, and fibrillary glomerulonephritis. In addition, tubulointerstitial diseases with the deposition of monoclonal immunoglobulins or their components are constituted by light chain (myeloma) cast nephropathy, light chain associated Fanconi's syndrome (light chain proximal [crystal] tubulopathy), and crystal-storing histiocytosis. In the present review article, we demonstrate the clinicopathological characteristics of MIDD, which is one of the representative diseases of plasma cell dyscrasias, and discuss various renal diseases with the deposition of monoclonal immunoglobulins or their components in glomeruli and the tubulointerstitium. We recommend that these renal diseases are arranged as one disease category, "renal diseases with deposition of monoclonal immunoglobulins or their components", in order to simplify the understanding of complicated diseases in plasma cell dysplasia.

Wstęp Gammapatia monoklonalna o znaczeniu nerkowym (monoclonal gammapathy of renal significance -MGRS) to jednostka charakteryzująca się wystąpieniem uszkodzenia nerek w związku z nefrotoksycznym działaniem białka monoklonalnego powstającego w przebiegu gammapatii monoklonalnej spełniającej kryteria MGUS (MGUS-monoclonal gammapathy of undetermined significance) [1].U większości pacjentów rozpoznanie MGUS nie stanowi wskazania do wdrożenia terapii cytotoksycznej, ponieważ przebieg schorzenia przez wiele lat bywa bezobjawowy. W przypadku MGRS wskazaniem do wdrożenia tego typu terapii jest patogenność produkowanego białka monoklonalnego, a celem leczenia zachowanie lub odzyskanie czynności nerek, a niekiedy także innych narządów [2]. Cel opracowaniaCelem pracy jest przedstawienie współczesnej wiedzy na temat gammapatii monoklonalnych o znaczeniu nerkowym w oparciu o dostępne dane w piśmiennictwie. MGUS i MGRS: kryteria rozpoznaniaKryteria rozpoznania MGUS obejmują: obecność białka monoklonalnego w jednej z następujących klas: IgG, IgA i IgM w stężeniu mniejszym niż 30 g/l, odsetek plazmocytów klonalnych w szpiku nieprzekraczający 10% i niewystępowanie cech patologii narządów zależnej od gammapatii [3, 4]. MGUS jest uważane za stan przednowotworowy wyprzedzający pojawienie się szpiczaka plazmocytowego, makroglobulinemii Waldenströma, amyloidozy AL lub innych nowotworów limfoproliferacyjnych. Termin gammapatia monoklonalna o znaczeniu nerkowym (MGRS) został wprowadzony w 2012 roku przez grupę ekspertów działających w ramach międzynarodowego zespołu: International Kidney and Monoclonal Gammapathy Reseach Group [1]. Pojęcie MGRS obejmuje schorzenia nerek (niekiedy dodatkowo także innych narządów) wywołane przez obecność monoklonalnego białka (immunoglobuliny lub jej fragmentu), uwalnianego przez klon plazmocytów lub limfocytów B. Z definicji pacjenci z MGRS nie spełniają kryteriów rozpoznania szpiczaka plazmocytowego ani chłoniaka z komórek B, występujące u nich zaburzenie hematologiczne odpowiada natomiast definicji MGUS [3, 4]. Diagnostyka hematologiczna powinna obejmować między innymi wykonanie elektroforezy białek osocza krwi oraz moczu, badania immunofiksacji krwi i moczu oraz określenie stężenia wolnych łańcuchów lekkich w surowicy krwi, a także badania szpiku z oceną cytologiczną, histopatologiczną oraz fenotypowanie komórek z oceną ich klonalności. W przypadku podejrzenia MGRS, szybkie i właściwe scharakteryzowanie gammapatii monoklonalnej i towarzyszącej jej nefropatii jest kluczowe dla rokowania.
