[Purpose] The purpose of this study was to analyze approaches and interventions with occupational performance in patients with stroke. [Subjects and Methods] In this study, articles published in the past 10 years were searched. The key terms used were “occupational performance AND stroke” and “occupational performance AND CVA”. A total 252 articles were identified, and 79 articles were selected. All interventions were classified according to their approaches according to 6 theories. All interventions were analyzed for frequency. [Results] Regarding the approaches, there were 25 articles for studies that provided high frequency interventions aimed at improving biomechanical approaches (31.6%). This included electrical stimulation therapy, robot therapy, and sensory stimulation training, as well as others. Analysis of the frequency of interventions revealed that the most commonly used interventions, which were used in 18 articles (22.8%), made use of the concept of constraint-induced therapy. [Conclusion] The results of this study suggest an approach for use in clinics for selecting an appropriate intervention for occupational performance.

Objective:The self-decisions of the client regarding the meaningful work as a therapeutic approach of client-orientation. The Cognitive Orientation to daily Occupational Performance (CO-OP) is an occupation-oriented problem-solving approach. The purpose of this study was to describe the goals and intervention protocols of CO-OP in those affected by stroke. Design: A systematic review. Methods: Using EBSCOhost, PubMed, and ProQuest databases, we searched studies published in the past decade that utilized the CO-OP intervention. An initial search revealed 71,171 potential articles. After applying our search criteria to screen the titles, abstracts, and full-text, we included 7 articles that met our inclusion and exclusion criteria. In this study, we used the patient, intervention(s), comparison, outcome method to analyze the 7 selected studies. We analyzed the frequency of goals and intervention protocols. Results: Seven articles met our selection criteria; these studies included participants with an almost normal cognitive function from inpatient and outpatient rehabilitation facilities. CO-OP was used for 237 goals; the most used goal was the instrumental activities of daily living. The training procedure used 3 types of self-selecting goals in the activities. One of the goals was not trained, but was only evaluated to determine the generation effect. The most common outcome measurements included the Canadian Occupational Performance Measure and the Performance Quality Rating Scale. Conclusions: This research provided information about the effectiveness of CO-OP and selecting the correct evaluation tool to assess the efficiency of the intervention. This study suggests that treatment with CO-OP in occupational therapy is effective and that it outlines common protocols.

IntroductionEvidence-based practice (EBP) is a global movement that fosters the use of research evidence, clinical expertise, the patient's values and circumstances, and information from the practice context. It aims to support high quality practice through providing and using sound evidence for decision making, thereby enhancing the client's outcomes. Occupational therapists have been challenged to advance the profession and adopt EBP as a global approach. Although EBP has been increasingly promoted and applied in occupational therapy over the last twenty years, there is a scarcity of research conducted in developing countries, and evidence-based occupational therapy is not yet a worldwide reality. Chile, a developing country graduating an increasing number of occupational therapists, provides a useful case for exploring EBP. The aim of this study was to investigate which sources of information are accessed by occupational therapists in Chile, and how knowledge is built and integrated in paediatric practice. MethodsInterpretive description served as the methodological framework for this study. Ten Chilean occupational therapists, who had diverse demographic profiles and worked in a variety of paediatric settings, were recruited. Participants completed in-depth interviews regarding their decision-making in daily practice. Interviews were conducted in Spanish, transcribed verbatim in Spanish, and then translated into English. Data were rigorously coded and analyzed thematically. To enhance credibility of the data analysis, preliminary conceptualizations and interpretations were returned to the participants for their critical DiscussionScarce access to research knowledge, precarious development of a research culture and lack of connection with the global scientific community can lead occupational therapists to rely on information that has not undergone quality evaluation. In Chile, the absence of regulatory bodies represent an additional risk to the profession , as there are no formal procedures to oversee the quality of professional practice, the standards of continuing professional development nor educational programs. Practitioners may benefit from developing a clinical reasoning framework based on evidence to guide their decision-making, in order to improve quality of practice and client outcomes. The establishment of a clinical reasoning framework based on evidence might be particularly relevant at this time, due to the increasing number of educational programs offering occupational therapy training in Chile.
