MRI findings in proven Mycobacterium tuberculosis (TB) spondylitis

Abstract: The incidence of skeletal TB is increasing. A better understanding of the MRI features of proven TB spondylitis in our setting is needed.Materials and methods. Histologically proven cases of TB spondylitis, with MR imaging performed at Universitas Hospital on a 1.5T scanner, were reviewed.Results. Typical findings of vertebral column involvement were seen in all patients, namely multiple levels affected and paravertebral abscesses. The thoracic spine was involved more than the lumbar spine. In nearly all cases … Show more

“…Many reports published to date indicate sparing of intervertebral discs in tuberculous spondylitis [ 23 , 24 , 31 , 32 ], which was linked to the fact that mycobacteria do not produce proteolytic enzymes. In our study intervertebral disc destruction was similar in both types of inflammation, which might be due to longer disease duration before MR imaging.…”

“…With the aid of intravenous administration of magnetic resonance contrast agents, MRI was highly accurate in distinguishing granulation tissue from cold abscess. [303132] MRI can reveal more extensive involvement than the plain films. [253334] MRI provided more exact anatomic localization of vertebral and paravertebral abscesses in multiple planes not previously available with more conventional diagnostic methods in the patients with suspected TB spondylitis.…”

“…Epidural abscess formation (figure 2) results in narrowing of the spinal canal diameter with resultant cord compression and neurological deficits. 5 In the present study, we have attempted to depict the various spectrum of presentations of spinal TB with clinical correlation.…”

“…10 Shanley DJ 6 evaluated radiographic manifestations of tubercular spondylitis like intraosseous and paraspinal abscess formation as seen in our study mentioned in Figure 4 & 6 and Figure 8, paraspinal abscesses in the lumbar region gravitate along the psoas sheath which can extend to the femoral region and cause erosion of overlying skin. 5,6 MRI is the gold standard of imaging in TB Spondylitis due to its superior soft tissue resolution and multiplanar capability. The classic pattern of spread starting anteriorly and moving to involve opposing vertebrae via subligamentous spread is clearly seen on MRI.…”
