“…Demonstration of a proximal hypoplastic BA below the abnormal communication, with enlargement above, has been considered an ancillary finding of PPTA (10). According to the hemodynamic of the posterior circulation distal to the connecting point between the PPTA and the BA, the BA can show a varying degree of hypoplasia of the proximal BA with or without hypoplasia of vertebral arteries (11).…”

“…In our case, the ipsilateral posterior communicating artery was absent and the ipsilateral vertebral artery was hypoplastic. Athale et al have described a proximal hypoplastic basilar trunk below the anastomosis, which enlarges to a more normal caliber above the aberrant communication, as an important ancillary sign of PTA and have opined that this should be looked for on routine MR studies in cases with suspected carotid‐basilar anastomoses 10 . In our case, the basilar trunk below the anastomosis was smaller in caliber than the post‐anastomotic segment.…”

“…Athale et al reported two women (46 and 19 y old), finding the classic appearance of persistent trigeminal artery on MR imaging [8]. This finding also was used as an ancillary sign of this anomaly in subtle cases of persistent fetal carotid-basilar artery anastomoses.…”
