Multifocal intraocular lenses in the setting of glaucoma

Abstract: Firm recommendations cannot be made on the use of multifocal intraocular lenses in patients with glaucoma as large studies have not been done to evaluate their benefit. Currently, the decision of whether to use multifocal intraocular lenses in patients with glaucoma has to be made on an individual basis.

“…To make matters more difficult, there is a paucity of data on the subject [171]. In what is currently the only study to assess multifocal IOLs in patients with eye disease, Kameth et al [172] study 133 eyes in 111 patients, 29 of them had either glaucoma or ocular hypertension.…”

“…Multifocal implantation may significantly decrease contrast sensitivity due to the IOL's light dispersion mechanics, especially in low mesopic conditions (low but not quite dark lighting situations) [11]. Thus, patients who have compromised contrast sensitivity, such as advanced glaucoma [12] or maculopathies, may be contraindicated for MfIOL use.…”

“…Both have been used in glaucoma patients with good results. 48 Aspheric IOL The newer aspheric lens may have a theoretical advantage in a glaucomtous eye. Glaucoma may preferentially affect contrast sensitivity as compared with visual acuity (an important visual function for daily activity), and patients with glaucoma often complain of ''poor'' vision despite their normal or near-normal Snellen visual acuity.…”
