Multiple Simultaneous Rare Distant Metastases as the Initial Presentation of Papillary Thyroid Carcinoma: A Case Report

Abstract: Papillary thyroid carcinoma (PTC) commonly metastasizes to regional lymph nodes. However, they infrequently cause rare distant metastases (RDMs), with the exclusion of lungs and bone metastases. RDMs are seldom identified prior to a primary thyroid cancer diagnosis. Therefore, cases initially presenting with synchronously multiple RDMs from PTC are extremely infrequent. This is a rare case of a 48-year-old man with initial diaphragm, pancreatic, and liver tumors from PTC. Following resection of the tumors, an … Show more

“…Previously reported cases treated with sorafenib have shown shortly delayed disease progression but unclear survival benefit in patients with liver and pancreatic metastases from advanced thyroid cancer [31,32]. However, in a recent case report of a PTC patient with multiple simultaneous metastases to the pancreas, kidney, gluteus maximus muscle, and lung who was treated with lenvatinib, SD was observed during follow-up 13 months after starting lenvatinib [33]. In our series, five patients received TKI therapy and four patients with lenvatinib showed a treatment response.…”

“…To date, the best therapeutic option for PTC with distant metastases is resection of metastatic tumour along with to-tal thyroidectomy, followed by radioacive iodine ablation therapy. [30] 4. CONCLUSION Our cases are rare presentations of PTC as a solitary metastatic lesion at unusual sites without involvement of commonly affected organs like lung or bone.…”

“…These tumors mainly invade lymphatic vessels and generate regional lymphatic metastases [ 100 , 104 ]. Distant metastases are infrequent, although lung and bone metastases have been described, and their development is associated with a poor diagnosis [ 105 ].…”
