Myofibroblastoma of the Breast: Literature Review and Case Report

Abstract: Myofibroblastoma of the breast is a rare benign spindle cell tumor. The main aim of this study is to review the literature of this rare tumor. We present a case of a mammary myofibroblastoma occurring in an 82-year-old man, emphasizing the clinical, radiological, and pathological features. The tumor was successfully identified and managed in our hospital. We would like to draw the attention of clinicians to myofibroblastoma as a rare possibility in the differential diagnosis of a breast mass.

“…An association with gynecomastia has been reported but was not present in this patient [8]. Myofibroblastomas have also been reported at surgical scar sites following breast cancer excision [9], which may be secondary to the migration and transformation of fibroblasts to the tissue injury location [10]. Imaging features of myofibroblastoma are nonspecific.…”

“…On mammography, myofibroblastomas are usually dense masses 1 to 4 cm in size, well defined, and with a round to ovoid shape (Figure A). On ultrasonography, they may show as homogeneously hypoechoic masses like fibroadenomas, or heterogeneous echoic masses such as hamartomas . In our case, a well‐circumscribed, heterogeneously hypoechoic mass was observed (Figure , B–D).…”

“…25 In histopathology, myofibroblastoma is presented as some bipolar spindle-shaped cells showing variable myogenic and fibroblastic differentiation in short intersecting fascicles interrupted by keloidal like eosinophilic collagen bands. 25,26 In immunohistochemistry, myofibroblastoma stains positively for Vimentin, Desmin, and CD34. Positively stained with estrogen, progesterone, androgen receptors, SMA, S100, bcl-2 and negatively for CD31, and cytokeratin are detected in some types of myofibroblastoma.…”
