Nationwide Trends of Hospital Admission and Outcomes Among Critical Limb Ischemia Patients

Abstract: While CLI admission rates have remained constant from 2003 to 2011, rates of surgical revascularization have significantly declined and endovascular revascularization procedures have increased. This has been associated with decreasing rates of in-hospital death and major amputation rates in the United States. Despite multiple adjustments, endovascular revascularization was associated with reduced in-hospital mortality compared to surgical revascularization during 2003 to 2011.

“…6 This is despite an increase in the prevalence of comorbidities such as diabetes mellitus, chronic kidney disease, and obesity. 6 Beyond better tools and techniques, many other challenges remain. These include proper reimbursement for treating complex lesions and patients, underutilization of revascularization in patients undergoing amputation, 22 and continued disparities in revascularization and amputation among blacks and those of lower socioeconomic status.…”

“…In Germany, there are an estimated 4.5 million people aff ected by PAD [5,6] and this fi gure is expected to rise. Compared to historical data [7], the rate of major amputation and amputation-related deaths has decreased over the last decades [8][9][10]. However, the overall prognosis of patients with PAD, particularly of the subset with critical limb ischaemia (CLI), remains poor [11].…”
