New concepts for the treatment of male lower urinary tract symptoms

Abstract: New concepts on BPH cause show interesting approaches, but are still in early stages. They carry, however, the highest prospect to bring up causal treatments. Some new compounds related to LUTS are on the verge to switch 'from bench to bedside'.

“…BPH is mainly a histological diagnosis, the clinical counterparts of which are benign prostate enlargement, and benign prostate obstruction (Abrams, ). The two clinical entities are characterized by a progressive development of symptoms (lower urinary tract symptoms, LUTS) – that are only partially derived from the prostate enlargement – which can affect quality of life and sexual function (Corona et al ., ; Gacci et al ., , ; Vignozzi et al ., ; Füllhase & Hakenberg, ). BPH‐associated LUTS are probably the most common urological conditions of aging men, with an estimated overall prevalence of 10.3%, and an overall incidence rate of 15 per 1000 males per year with even higher rates in older individuals (Verhamme et al ., ).…”

Medical treatment is mainly based on the use of alpha‐blockers and 5‐alpha reductase inhibitors (5ARIs; Füllhase & Hakenberg, ; Yuan et al ., ). More recently, phosphodiesterase type 5 inhibitors (PDE5i), and tadalafil in particular, have been demonstrated to improve LUTS/BPH, although their role in reducing obstructive symptoms is more conflicting (Gacci et al ., ).…”

“…However, other investigators state that, apart from the old concept of mainly hormone-dependent prostate growth, new ideas about the cause of BPH include an inflammatory factor [22]. Therefore, materials presented at a satellite symposium entitled, “Inflammation and Prostatic Diseases: From Bench to Bedside,” held during the 2015 annual meeting of the European Association of Urology in Madrid, Spain, revealed that reducing chronic inflammation is a target for the treatment of BPH [23].…”

“…Conflicting results from several studies evaluating the role of anti-inflammatory drugs on BPH and LUTS are available in the literature, and thus clear recommendations cannot be drawn. 46 Moreover, a lot of available studies are characterized by their relatively short duration and lack of randomization and control group, and as such no high-quality data can be extracted from their analysis. 32 …”
