New Suturing Techniques to Reconstruct the Keystone Area in Extracorporeal Septoplasty

Abstract: BACKGROUND: Severe septal deformations require adequate treatment to restore shape and function using extracorporeal septoplasty. Because it has been criticized for being technically demanding to execute and has increased risk for aesthetic complications, the authors have developed two new suture techniques for refixation of the neoseptum. METHODS: A retrospective analysis of multisurgeon consecutive extracorporeal septoplasties performed from January of 2014 to December of 2014 was conducted at a single insti… Show more

“…12D) and proximal solid fixation to the upper lateral cartilages and especially, as more recently described, to the nasal bones. 13 Many technical nuances exist here, regarding shaping the spine appropriately, for instance, by piezo or electric tools, as well as best configurating transosseous sutures between the Fig. 6 A frequent application for rib segments is to splint and extend the remaining septum so that a stable L strut is achieved.…”

“…Another more complicated but highly effective option for internal fixation of the bony pyramid is the transcutaneous transosseous cerclage suture (TTCS) as described by Rezaeian and coworkers. 33 Like the crisscross suture, the TTCS was developed to reattach the neoseptal construct following extracorporeal septoplasty and greatly enhances stabilization of the aggressively mobilized bony pyramid.…”

“…9 A major drawback of this technique is destabilization of the junction of the quadrangular cartilage and bones (keystone area), with resultant requirement for unique forms of fixation, such as percutaneous sutures, or sutures to the ethmoid or nasal bones. 13 However, even in the most experienced of surgeons, notching or saddling may occur as a result of keystone instability. 9 ASR has been developed to replace the extracorporeal septoplasty technique to minimize destabilization of the keystone (and thus preserve dorsal contour).…”
