Hydrops fetalis is defined by the accumulation of fluids in the serous membranes (pleurisy, ascites, pericarditis...), it is a rare condition; its etiologies are divided into immunological and non-immunological. Supraventricular tachycardia remains a known cause, non-immunological, but difficult to manage. In this article we report the case of a newborn with hydrops fetalis secondary to supraventricular tachycardia, which failed to be treated. The clinical, paraclinical, etiological diagnosis and finally the treatment were discussed.

Drug ineffectiveness: case reportA female neonate on the first day of her life exhibited drug ineffectiveness to adenosine during treatment for paroxysmal supraventricular tachycardia (PSVT).The female, neonate who was born by caesarean section at 33 + 1 weeks of gestation, had worsening hydrops foetalis (HF). At delivery, the clinical and ECG findings were suggestive of PSVT with narrow complexes. She started receiving vagal manoeuvres and adenosine five boluses at a dose of 250 µg/kg/dose [route not stated], but were ineffective.The neonate received treatment with amiodarone and was responsive. Echocardiography revealed dilated right cardiac chambers and severe tricuspid insufficiency. She further transient experienced PSVT episodes, but these were gradually improved with amiodarone, spironolactone, hydrochlorothiazide and furosemide. A final diagnosis of primitive tricuspid valvulopathy with heart failure secondary to PSVT was made. She showed significant improvement and was discharged later.
