Tracheot omy is performed in cases of upper airway obstruction or chronic pulmonary disorders. The Tracheotomy Speech Valves (TSV) improve communication and airway hygiene and humidification of tracheotomized patients. Aim: To show the low cost Brazilian TSV and its use in speech rehabilitation of tracheotomized patients, to evaluate diaphragm opening resistance and comfort to the patient. Study Design: Experimental, contemporary cohort . Materials and methods: The TSV was used in 32 patients. The valve has a diaphragm within a stainless steel body with plastic fittings. We studied the level of respiratory comfort according to the degree of valve diaphragm resistance, 40, 50 and 60 shores. Results: All the patients used the TSV coupled to the cannula in a regular basis, 26 of them did it for more than 12 hours daily and from these, 14 used it for 24h daily. The diaphragm pressure obtained was that of 40 shores for 13 patients and 50 shores for 19 patients. 60 shores was never used. Conclusion: the metal TSV helps with speech without the need for closing the cannula with one's finger, and breathing was comfortable. We achieved standard diaphragm resistance. Currently all the patients from this study use this TSV with speech and 43.75% use it full time.

A comunicação do paciente traqueostomizado: uma revisão integrativa

It is an integrative review aiming at knowing the scientific production and analyze the strategies on the communication of the tracheostomized patient. The search on the site of the Virtual Health Library (Biblioteca Virtual em Saúde -BVS), with the descriptors "communication and tracheostomy", resulted in publications of the Latin American and Caribbean Health Sciences Literature (Literatura Latino-Americana e do Caribe de Informação em Ciências da Saúde -LILACS), International Literature (MEDLINE), and Coleciona SUS databases. Scientific articles published between 2001 and 2015 were considered, as they represented the period of the greatest scientific production involving adults, and unrestrictedly for Spanish, English and Portuguese idioms or the type of study. Sixteen (16) publications, comprising 15 articles and 1 thesis, were selected, which were classified by categories: knowledge, communication and supporting resources. From the analyzed publications, it is verified that the concern on the communication of tracheostomized patients has been an increasing theme in the context of the scientific production in the past decade. Keywords: Communication; Tracheostomy; Adult RESUMO Trata-se de revisão integrativa com objetivo de conhecer a produção científica e analisar as estratégias acerca da comunicação do paciente traqueostomizado. A busca no portal da Biblioteca Virtual em Saúde (BVS), com os descritores "communication and tracheostomy", resultou em publicações das bases de dados Literatura Latino-Americana e do Caribe de Informação em Ciências da Saúde (LILACS), Literatura Internacional (MEDLINE) e Coleciona SUS. Foram considerados os artigos científicos publicados no perí-odo de 2001 a 2015, por representar o período com maior produção científica, que envolvessem adultos e, sem restrição quanto aos idiomas espanhol, inglês e português nem para o tipo do estudo. Foram selecionados 16 publicações, composta por 15 artigos e uma tese, as quais foram classificadas por categorias: conhecimento, comunicação, e recursos auxiliares. Diante das publicações analisadas, verifica-se que a preocupação acerca da comunicação do paciente traqueostomizado é tema crescente no contexto da produção científica nessa última década.

Objetivo : conhecer os critérios utilizados pelos fonoaudiólogos na avaliação clínica do paciente traqueostomizado no leito hospitalar e/ou internamento domiciliar, da cidade de Salvador, Bahia. Métodos : trata-se de um estudo transversal, descritivo, de abordagem quantitativa, cuja população constituiu-se de 28 fonoaudiólogos, que contribuíram respondendo a um formulário contendo 19 questões sobre o tema Critérios para Avaliação Clínica Fonoaudiológica do Paciente Traqueostomizado no Leito Hospitalar e/ou Internamento Domiciliar (home care). Resultados : no presente estudo, em relação aos critérios selecionados primariamente para a prática clínica, os resultados foram: 26 (92,9%) estabilidade clínica; 24 (85,7%) nível de consciência; 24 (85,7%) estado de alerta; 14 (50%) capacidade de proteção de vias aéreas; 9 (32,1%) estado nutricional; 9 (32,1%) tosse; e 6 (21,4%) possibilidade de receber via oral. Conclusão : ao término do estudo realizado foi possível concluir que os critérios para avaliação fonoaudiológica no paciente traqueostomizado no leito, na cidade de Salvador, Bahia, em sua maioria, encontram-se de acordo com os critérios descritos na literatura. Contudo, existem poucos estudos que estabelecem critérios para a decanulação, portanto, ressalta-se a necessidade de validações de protocolos para uma melhor atuação do processo de decanulação desses pacientes.
