Novel approach to combined defaecation and micturition disorders with rectovaginovesicopexy

Abstract: RVVP provides satisfactory improvement of combined defaecation and micturition disorders. The benefits of a limited extension of the RVP seem to outweigh potential side-effects such as deterioration or de novo defaecation or micturition disorders.

“…The symptoms of enterocele include pelvic pressure, obstructed defaecation, lower abdominal pain and faecal incontinence [2] . Several surgical procedures by the transvaginal or abdominal approach have been suggested to improve symptoms in these patients [3–8] . Transvaginal procedures decrease symptoms in more than 80% of patients, but decrease vaginal lumen and depth, and induced more relapse than transabdominal approaches [3,5,6,8,9] .…”

“…This was observed in 30% of the patients preoperatively and disappeared after surgery in the present study. This might be another effect of restoring a firm rectovaginal septum with the procedure as in the case of a similar mechanism with rectovaginovesicopexy [20]. Transvaginal anterior levatorplasty with posterior colporrhaphy might be an option for symptomatic rectocele to improve anorectal and urinary dysfunctions with morphological disorders.…”

“…The literature contains many reports documenting improvement in defecatory symptoms [8][9][10], but only a few of these mention new-onset symptoms and postoperative dyspareunia. In our clinic, the modified anterolateral rectopexy described by Silvis et al [11] is used for treatment of rectocele.…”
