Novel cytomorphology of the giant proerythroblasts of parvovirus B19 infection

Abstract: The morphology of the giant proerythroblasts (GPE) in air-dried and Wright-Giemsastained smears of bone marrow in 16 patients with pure red cell aplasia (PRCA) caused by parvovirus B19 infection is described. B19 infection was diagnosed by the presence of the virus or viral DNA and/or IgM antibodies. Twelve patients had chronic hemolytic anemia and aplastic crisis and 4 patients had AIDS with chronic PRCA. In patients with chronic hemolytic anemia and aplastic crisis, GPE were not detectable in bone marrow bio… Show more

“…The diagnosis of B19 infection may also be hinted at by the histopathological aspect of bone marrow: overall hypocellularity, and the presence of giant pronormoblasts with finely granulated cytoplasm and glassy, variably eosinophilic, intranuclear inclusions with a clear central halo (lantern cells) are thought to be indicative of parvovirus B19 infection 6 . These giant pronormoblasts were not described in our first patient, whose diagnosis resulted from the systematic dot-blot testing of persistently anemic AIDS patients.…”

“…Giant pronormoblasts are early erythroid cells with a diameter of 25 to 32 m, large eosinophilic nuclear inclusion bodies, and cytoplasmic vacuolization, and occasionally, "dogear" projections may be observed (as indicated in Fig. 4) (44,55,168,249). EM of cells reveals cytopathic ultrastructural changes that include pseudopod formation, marginated chromatin, and virus particles in the nucleus (378).…”
