Novel multidisciplinary approach to monitor and treat fetuses with gastroschisis using the Svetliza Reducibility Index and the EXIT-like procedure

Abstract: ObjectiveTo describe our initial experience with a novel approach to follow-up and treat gastroschisis in “zero minute” using the EXITlike procedure.MethodsEleven fetuses with prenatal diagnosis of gastroschisis were evaluated. The Svetliza Reductibility Index was used to prospectively evaluate five cases, and six cases were used as historical controls. The Svetliza Reductibility Index consisted in dividing the real abdominal wall defect diameter by the larger intestinal loop to be fitted in such space. The EX… Show more

“…This procedure involves a planned cesarean delivery with immediate gastroschisis repair while the umbilical cord and placental circulation are still intact 64,65 . Unlike the ex utero intrapartum treatment (EXIT) procedure, the EXIT‐like procedure does not require maternal general anesthesia or tocolysis 65 .…”

“…The article under analysis ( 2 ) is based on and is a recombination of several ideas that appeared over the last 20 years, which are still evolving and are far from their finalization. Gastroschisis, due to the difficulties in treatment and tumultuous progression, is a malformation favorable as testing ground of new ideas.…”
