Novel Musculoskeletal and Orthopedic Applications of 18F-Sodium Fluoride PET

Abstract: The bone-seeking properties of 18 F-sodium fluoride (NaF) were first described in 1962 by Blau and colleagues, 1 who used this positron-emitting radiotracer in skeletal scintigraphy. Both high uptake by bone and rapid plasma clearance of NaF were determined to be advantages in imaging skeletal metabolism. 2 The mechanism of NaF uptake relies on the exchange of OH À for 18 F À , converting hydroxyapatite on the surface of the bone matrix to fluorapatite. 2,3 One hour after NaF is administered intravenously, onl… Show more

“…For instance, correlations between 18 F-NaF uptake measured by SUV and age at the humeral shaft and lumbar spine have been reported to show opposite trends [ 49 ]. Overall, the validation of 18 F-NaF-PET for the treatment and diagnosis of osteoporosis with further studies will certainly enhance the way metabolic bone diseases are diagnosed and treated [ 7 , 50 ].…”

“…18 F-NaF-PET/CT shows great promise for early detection and monitoring of heterotopic ossification, or external growth of bone outside of the normal skeletal system [ 50 , 96 ]. Recently, the potential of 18 F-NaF-PET/CT in diagnosing and predicting the onset of heterotopic ossification in fibrodysplasia ossificans progressiva (FOP) has been gaining traction.…”

“…Vascular Micro-calcification Unlike the nonspecific nature of FDG uptake, NaF localizes specifically to areas of ongoing calcification [46]. Therefore, NaF-PET images are not affected by physiologic myocardial uptake and therefore can detect early signs of coronary plaque micro-calcification in atherosclerosis.…”

“…18 F-NaF binds minimally to protein and clears rapidly in the plasma, allowing for the precise and fast acquisition of image with low background uptake already within 45 to 60 min after administration (12,13). The role of 18 F-NaF-PET in diagnosis and clinical management of malignant bone diseases, benign osseous conditions, and atherosclerosis has been well-studied previously (14)(15)(16). While 18 F-NaF-PET has been employed to assess the sources of neck pain (8), its use for measuring age-related degenerative changes in the spine has not been fully explored.…”
