Novel Surgical Approaches to High-Grade Gliomas

Abstract: Treatment of patients with high-grade glioma (HGG) should begin with thorough evaluation by a specialized multidisciplinary team to determine whether or not the patient is appropriate for surgery, chemotherapy and radiotherapy. Particular attention should be paid to the performance status and neurological function. Surgery is the first step in therapeutic intervention. Patients undergo either biopsy, debulking surgery or maximal resection depending on the anatomical location of the tumour and the patient's cli… Show more

“…De Witt Hamer et al found that surgeries incorporating intraoperative stimulation monitoring had 2 times fewer late severe neurologic deficits (muscle strength <3, aphasia, severe dysphagia, hemianopsia, vegetative state) [35]. Intraoperative ultrasound can also be used in low‐grade gliomas for real‐time imaging, which allows immediate feedback regarding tumor location during surgery; use in high‐grade gliomas is limited due to edema and hemorrhage [36,37].…”

“…Existing therapeutic strategies for GBM include surgery, radiation and chemotherapy. Combined radiotherapy and chemotherapy are currently used for the cytoreduction of the tumor, but over 90% of patients usually experience rapid tumor recurrence [ 4 , 5 , 6 ]. Tumor recurrence, poor prognosis, and the side effects of radio- and chemotherapy suggest the need to prioritize the further search for novel therapeutic approaches, especially among natural sources.…”

“…However, overall prognosis remains nonideal. [5,6] For a long time, we have often emphasized that glioma patients should have reasonable medical treatment as biological entities, while ignoring their status as social beings. Socioeconomic status and psychological support also play important roles in their survival prognosis.…”
