Nutritional Inadequacy: Unraveling the Methodological Challenges for the Application of the Probability Approach or the EAR Cut-Point Method—A Pregnancy Perspective

Abstract: The aim of this study was to unravel the methodological challenges when exploring nutritional inadequacy, involving 608 healthy pregnant women. The usual intake of twenty-one nutrients was recorded by employing a validated FFQ. Simulated datasets of usual intake were generated, with randomly imposed uncertainty. The comparison between the usual intake and the EAR was accomplished with the probability approach and the EAR cut-point method. Point estimates were accompanied by bootstrap confidence intervals. “…The unique role of vitamins and minerals in enzymes (acting as co-factors) and transcription factors, during all stages of cell growth and differentiation, suggests that micronutrient imbalance could evoke alterations in maternal and fetal metabolism [ 55 ]. For instance, deficiencies in B-complex vitamins, which serve as coenzymes in the release of energy from macronutrients, may have an impact on cellular growth and nerve tissue development [ 56 , 57 ]. Furthermore, the synergistic action of methyl-donor nutrients, i.e., folate and vitamin B-12, is essential for the methylation of DNA, RNA, protein neurotransmitters and phospholipids.…”

“…The exclusion criteria for the study population are described in detail in a previous publication [ 56 ]. Briefly, of the initial population ( n = 673) enrolled in the present investigation, 48 women were excluded due to insufficient dietary information, medical complications and inconsistent answers.…”

“…EAR defines the level of nutrition that meets the daily needs of 50% of healthy individuals within a target group, and is obtained as the median of the requirement values [ 12 ]. The EAR cut-point method is widely used in nutrition research because it enables simple estimations to be made of the proportion of subjects consuming nutrients below required levels [ 18 ]. In a similar manner, nutrient intake above the tolerable upper intake level (UL) was considered as unsuitable.…”
