Opportunities and barriers in paediatric pulse oximetry for pneumonia in low-resource clinical settings: a qualitative evaluation from Malawi and Bangladesh

Abstract: ObjectiveTo gain an understanding of what challenges pulse oximetry for paediatric pneumonia management poses, how it has changed service provision and what would improve this device for use across paediatric clinical settings in low-income countries.DesignFocus group discussions (FGDs), with purposive sampling and thematic analysis using a framework approach.SettingCommunity, front-line outpatient, and hospital outpatient and inpatient settings in Malawi and Bangladesh, which provide paediatric pneumonia care… Show more

“…At a policy and planning level, our data support calls for the routine use of pulse oximetry in all neonates and children admitted to hospital, irrespective of diagnosis [22,55]. We know healthcare workers (including low-level community-based healthcare workers) are able to use pulse oximetry effectively [6,22,52,[56], [57], [58]]. However, introducing pulse oximetry into routine care is challenging and requires a multi-faceted approach (e.g.…”

“…As this patient group was slightly older, healthier and more agitated, it suggests these children may have been more mobile during testing, possibly accounting for the poorer performance. This emphasizes the importance of software that can account for motion, a key challenge that has been highlighted by healthcare providers in small children …”

“…Barriers to wider implementation include cost, lack of training and supervision, and lack of robust pulse oximeters and probes. In pediatric populations additional barriers include the lack of a high‐quality, reusable, low‐cost probe that fits all ages of children and is tolerant to movement …”

“…Our previous focus group work suggests that CHWs place a higher “value” on pulse oximetry, which may lead to more careful adherence to SpO 2 protocols, and highlights the enormous potential of community-based implementation programs for pulse oximetry. 9 , 24 …”
