Optimum number of oocytes for a successful first IVF treatment cycle

Abstract: Ovarian stimulation in IVF allows selection of embryos for transfer, but may have detrimental effects on oocyte and embryo quality and endometrial receptivity. This study investigated the optimal response to ovarian stimulation in terms of number of oocytes for achieving pregnancy in a first IVF cycle. Data from 7422 women who underwent their first IVF cycle for standard indications were analysed. All had been treated with exogenous gonadotrophins and gonadotrophin releasing hormone (GnRH) agonist co-treatment… Show more

“…The association between the number of oocytes retrieved and pregnancy rate is controversial. Some researchers have shown a correlation between the number of oocytes retrieved and higher pregnancy rates [50][51][52] whereas others argue that excessive response to ovarian stimulation might result in detrimental effects on oocyte and embryo quality and/or that supra-physiological estradiol levels may alter implantation [53,54]. Of note, we observed a trend towards a lower number of embryos frozen among cohorts with non-cleaved embryos, although the numbers were not statistically significant.…”

“…There were some differences in the number of retrieved oocytes when comparing the groups with a P/F ratio≤.075 with those with a ratio>.075, with a lower number of oocytes noted in the latter group. However, according to the literature [8,24], both groups of patients have been found to exhibit good prognoses considering the number of retrieved oocytes. When between 6 and 15 oocytes are retrieved, the pregnancy rates for one fresh ET are similar, and when oocyte numbers are either below or above that range, outcomes are compromised [25].…”

“…In the case of equal ovarian assessment, other parameters could influence pregnancy rate, for example age [18][19][20][21], which is the most important predictive variable of live birth in IVF. Globally, women who are poor responders have a lower pregnancy rate than normally responding women [19,[22][23][24]. Even in young poor responders IVF results are low but acceptable [25].…”
