Oral Focal Acantholytic Dyskeratosis

Abstract: Focal acantholytic dyskeratosis (FAD) is an uncommon lesion of the oral mucosa with only 22 cases reported in the medical/dental literature. Fifteen additional cases of solitary oral FAD are presented. Of these cases, 4 occurred clinically as leukoplakia. Leukoplakia-associated lesions may be an addition to the current classification of solitary FAD.

“…The etiology of oral WD/FAD is also unknown. Some authors have suggested a role for local trauma (10, 12), and others have considered a role for tobacco (4, 9, 11). Taken together, the data of the present analysis of the literature revealed that smoking appears to be the most prevalent predisposing factor, with about 60% of the 41 reported lesions appearing in smokers.…”

“…They are uncommon and except for one series (9), most reported cases are either single case reports or reports of 2-3 cases. Owing to the paucity of the reports, many aspects of the oral lesions have not been elucidated.…”

“…1 These variants may be observed not only in the classical dermatoses that coined the term but also in various other skin diseases such as Grover's disease, acantholytic acanthoma, 2 warty dyskeratoma, 3 linear epidermal naevus, 4 naevus corniculatus, 5 oral focal dyskeratosis 6 and acantholytic dermatosis localized to the vulvocrural area. 1 These variants may be observed not only in the classical dermatoses that coined the term but also in various other skin diseases such as Grover's disease, acantholytic acanthoma, 2 warty dyskeratoma, 3 linear epidermal naevus, 4 naevus corniculatus, 5 oral focal dyskeratosis 6 and acantholytic dermatosis localized to the vulvocrural area.…”

“…The four major patterns of acantholysis are mainly defined by the level of clefting and include: (i) the acantholytic dyskeratotic pattern of Darier's disease; (ii) acantholysis in the entire epidermis as observed in Hailey±Hailey disease; and (iii) a suprabasal (vulgaris); as well as (iv) a superficial (seborrhoeic) type of pemphigus pattern. 1 These variants may be observed not only in the classical dermatoses that coined the term but also in various other skin diseases such as Grover's disease, acantholytic acanthoma, 2 warty dyskeratoma, 3 linear epidermal naevus, 4 naevus corniculatus, 5 oral focal dyskeratosis 6 and acantholytic dermatosis localized to the vulvocrural area. 7 All patterns of acantholysis may also occur as an incidental histopathological finding, remaining subclinical in many inflammatory and neoplastic skin diseases, for example: leucocytoclastic vasculitis, 1 pityriasis rosea, chondrodermatitis nodularis helicis, 8 dermatofibroma, 9 basal cell carcinoma 1 and also in acral lentiginous malignant melanoma.…”

“…Tlie specfrum of disorders with fhe histopathologic pattern of localized aeantholytic dyskeratosis has also been enlarged in the last few years. Formerly, they were limited to classic warty dyskeratoma, a localized form of acanfliolytic dyskeralosis involving to the infundibular epithelium (8), but foei of localized acantholytic dyskeratosis have also been described invoh ing the oral mucosa (oral focal acantholytic dyskeratosis) (11). Brownstein found acantholytic patterns of pemphigus \ tilgaris, pemphigus vegetans, superficial pemphigus, and Hailey-Hailey disease in a series of 31 solitary keratotic papules or nodules.…”
