Orbital cellulitis following silicone-sponge scleral buckles

Abstract: BackgroundAcute or chronic infection of the scleral explant is rare. We report seven cases of scleral explant infections that caused orbital cellulitis.Materials and methodsThis was a retrospective chart review of oculoplastics at oculoplastics and vitreo-retinal units in a secondary referral hospital. All subjects had orbital cellulitis secondary to scleral buckle in the range of January 1990 to March 2010. Demographics, imaging studies, and pathology specimens were reviewed.ResultsA total of 841 silicone-spo… Show more

“…Orbital cellulitis has been associated with silicone-sponge buckles; however, cellulitis is uncommon after buckle removal. 15 Infectious disease literature recommend lengthy antibiotic treatment greater than 10 weeks. 14 The challenge in managing this patient's SB-related NTM ocular infection was in deciding when to discontinue systemic antibiotics, particularly given concern for the orbital cellulitis.…”

“…Scleral buckle elements have characteristic CT findings. Solid silicone bands appear hyperdense on CT, but silicone sponges appear hypodense [ 2 , 8 , 12 ]. The density of the sponge is similar to air because of the air pockets within the material, with an attenuation of −250 to −400 Hounsfield units [ 8 ].…”

“…Orbital cellulitis is an unusual complication of SB 7 . We report a case of orbital cellulitis after a SB procedure that was diagnosed and clinically treated at our hospital with good outcomes and we provide a revision of the pertinent literature.…”
