Orthodontic space closure after first molar extraction without skeletal anchorage

Abstract: Side effects during mesialization of the second molars without skeletal anchorage in the anterior dentition/PM were observed primarily affecting the incisors integrated into the anterior anchorage unit. These side-effects resulted in posterior displacement of the soft tissues, including a change in profile. This must be taken into consideration when taking this therapeutic approach.

“…13,14,21 It is also possible to achieve good results following the removal of these teeth using fixed appliances, although treatment times tend to be increased and appropriate mechanics need to be used. 33,34 It is not advisable to extract a healthy premolar for orthodontic purposes if the first permanent molar in the same quadrant is heavily restored.…”

“…Space closure should be planned such that the midlines remain coincident and the arches should not get skewed maintaining a Class I buccal segment relation bilaterally. [7] The space closure objectives should establish the posterior occlusion before finishing the anterior segment as predetermined on a Kesling setup. [8] The final occlusion should be aimed such that it provides a mutually protected occlusion.…”

“…The orthodontic-only (without SA) mesialization of posterior teeth is difficult, if not impossible to achieve without secondary effects on the teeth positioned anteriorly to the space. 1 The use of SA allows for stabilization of anterior teeth while posterior teeth are being pulled mesially, as an indirect source of anchorage, or allow for direct traction of the posterior segments. [2][3][4] It is important to note that even with SA, mesial movement of posterior segments is slow and difficult.…”
