Osteochondritis dissecans of the trochlea of the humerus

Abstract: Osteochondritis dissecans is a transchondral fracture. Localization to the elbow is rare when it is usually located in the capitellum of the humerus or the radial head. Osteochondritis of the trochlea of the humerus is described in a 12 year old X-ray and MRI.

“…Loose bodies were identified on MRI in three patients but were found at surgery in only two [8]. In one patient with OCD of the humeral trochlea the lesion had a low signal on both T1-and T2-weighted images, and displacement of the fragment was not seen [9]. In none of these studies was intravenous administration of gadopentetate dimeglumine performed.…”

“…Although experiences with MRI of pathologic conditions of the elbow have been reported [7,8], MRI findings in osteonecrosis and OCD of the elbow have been described in only a few cases [6][7][8][9]. Murphy [8] reported three cases of OCD of the capitulum with defects of low signal intensity on T1-weighted images.…”

“…There are some rare cases involving the trochlea. 8,12,13 This may reflect biomechanical stresses and vascular anatomy of the elbow. The radiocapitellar joint is a secondary stabilizer of the elbow and accepts up to 60% of the compressive axial load in extension 1 .…”

“…There are only a few reports of OCD in the trochlea of the humerus. 9,17,28 The ideal goal of treatment for OCD lesions is to resurface the osteochondral defect with hyaline cartilage and to prevent the occurrence of osteoarthritis. Recently, to achieve this surgical goal, autologous chondrocyte implantations (ACIs) or tissue-engineered cartilage transplantations (TCTs) have been performed for OCD lesions of the knee and the ankle.…”
