Osteoid Osteoma of the Trapezium: Case Report of an Unusual Tumor Location Presenting a Diagnostic Challenge

Abstract: Osteoid osteoma is a benign bone tumor, which represents approximately 10% of all benign bone tumors. When localized to the carpus, osteoid osteomas are most often seen in the scaphoid and capitate. Rarely, these tumors can also be observed in the trapezium. Given the infrequency with which osteomas are located in the trapezium and often nonspecific presenting symptoms, diagnosis of this tumor can be challenging and requires a high index of suspicion.

“…The patient was treated with curettage, experiencing immediate improvement in pain and no recurrence at 12-month follow-up. A 34-year-old woman with osteoid osteoma was initially misdiagnosed with carpometacarpal arthritis, according to a report by Roberts et al [6] in 2017. In this instance, an MRI scan revealed a hypointense circular lesion along the dorsal portion of the trapezium.…”

“…Reviewing the literature, we found only seven osteoid osteomas reported at the trapezium. The patients typically complain of pain worsening during the night that may also be intense enough to awaken them [3,5,6]. A computed tomography (CT) scan is the imaging of choice to reach a diagnosis, considering plain radiographs may be inconclusive.…”

“…After excision, the patient experienced complete pain relief and did not have any recurrence. 39 In this study, because of the unusually large tumor size of 1.3 cm, we filled the bone loss with bioactive glass to avoid donor morbidity in such a young patient.…”

“…The patient experienced immediate improvement in clinical pain and no recurrence at a 1-year follow-up. In 2017, Roberts et al 39 described a case of a 34-year-old woman with osteoid osteoma initially confused with carpometacarpal arthritis. In this case, an MRI scan showed a hypointense circular lesion along the dorsal aspect of the trapezium, and a CT study was conducted to better characterize the lesion.…”
