Outcome following repair of 63 sagittal fractures of the proximal phalanx in UK Thoroughbreds using either a triangular or linear screw configuration

Abstract: Background A triangular screw configuration has been suggested as preferable for repair of sagittal fractures of the proximal phalanx. Objective To assess the outcome of a triangular screw construct for repair of incomplete and complete minimally displaced proximal phalanx fractures under standing sedation in a population of Thoroughbred racehorses, compared with a cohort repaired using a linear screw configuration. Study design Retrospective cohort study. Methods Medical records and radiographs were accessed … Show more

“…However, together with the median fracture gap width of R‐2 and unloaded R‐2 constructs, it supports previous conclusions of dynamic fracture gap instability in linear repairs 10 . Recent clinical evidence associating triangular as opposed to linear repairs with a faster return to racing echoes this conclusion 9 …”

“…Incomplete parasagittal P1 fractures provide an ideal opportunity to perform standing repairs because few implants are required and fracture gap instability is not a concern. Nevertheless, implants are best tightened with the leg unloaded to counteract the potential distraction with weight bearing 9,11,12 . While our results provide evidence of a clear absence of effect and that limb elevation is not required, aspects of study power and the closer clinical context must be considered in light of negative findings.…”

“…По данным мировых ученых, большинство лошадей с сагиттальными переломами проксимальной фаланги лечат с помощью фиксации стягивающими винтами [8]. Так указывается, что треугольное расположение винтов при скреплении неполных и полных сагиттальных переломов с минимальной репозицией костных отломков более эффективно, чем линейное.…”
