Purpose. To assess the efficacy, safety, and predictability of the Visian Implantable Collamer Lens (ICL) model having a central port in patients over 40 years of age. Methods. This study included 33 eyes from 21 patients who underwent V4c ICL implantation for the correction of myopia and myopic astigmatism. We assessed uncorrected (UDVA) and corrected (CDVA) distance visual acuity, refraction, intraocular pressure (IOP), endothelial cell density (ECD), vault, and adverse events occurring over a 1-year period. Results. Mean age of the patients at the time of implantation was 43.52 ± 4.49 years (range: 40 to 56 years). Efficacy and safety indexes were 1 and 1.09, respectively. Surgical outcomes for CDVA were as follows: no eye lost any lines, 19 eyes (57.58%) showed no CDVA changes, 7 eyes (21.21%) gained 1 line, 4 eyes (12.12%) gained 2 lines, and 3 eyes (9.09%) gained ≥3 lines. Mean postoperative spherical equivalent (SE) was −0.09 ± 0.47 D. A total of 29 eyes (87.8%) were within ±0.50 D and 31 eyes (93.9%) were within ±1.00 D of the desired SE. At 1-year, mean IOP was 15.27 ± 3.03 mmHg (range: 9 to 20 mmHg, p=0.12 pre vs. post) and mean ECD was 2516 ± 234 cells/mm2 (p=0.29 pre vs. post). Mean postoperative vault was 320 ± 136 μm, with 201–300 μm being the most prevalent vault range for 9 eyes (31.03%). None of the eyes showed a vault >701 μm. There were neither intraoperative nor postoperative complications; in fact, all ICL implantation procedures were uneventful. Conclusions. Our study’s findings support the use of this lens in patients over 40 years of age. A long follow-up period is advisable to monitor ICL position relative to the crystalline lens.

Purpose To investigate two-year outcomes of Visian Implantable Collamer Lens with a central hole (ICL V4c) implantation for correcting high myopia. Methods Sixty-one eyes of 32 patients went through ICL V4c implantation. Safety, efficacy, predictability, and intraocular pressure were evaluated 2 years postoperatively. Anterior chamber volume (ACV), anterior chamber depth (ACD), anterior chamber angle width (ACAW), and vault were measured using a Scheimpflug tomography imaging system. Results At 2 years, the spherical equivalent refraction decreased from preoperative −14.62 ± 4.29 D to −0.90 ± 0.95 D, with 79% of the eyes within ±0.50 D and 98% within ±1.00 D of the intended correction. The efficacy index was 1.03 ± 0.23, and the safety index was 1.24 ± 0.26. Corrected distance visual acuity (CDVA) remained unchanged in 23% of the eyes, 75% gained one or more lines of CDVA, and 2% lost one line. The ACV, ACD, and ACAW, respectively, decreased from 193.28 ± 29.15 mm3, 3.15 ± 0.23 mm, and 36.51 ± 6.54 degree to 112.48 ± 17.01 mm3, 2.99 ± 0.23 mm, and 22.54 ± 5.27 degree (p=0.0008, 0.008, and 0.0003, resp.). Intraocular pressure was 15.39 ± 2.88 mmHg before surgery and was 15.86 ± 4.11 mmHg at 2 years (p=0.11). Conclusion Implantation of ICL V4c is a safe, effective, and predictable procedure for correcting high myopia. Reduction of anterior chamber space after surgery did not induce intraocular pressure increase during the 2-year follow-up.

Wprowadzenie. Coraz więcej pacjentów z refrakcyjnymi wadami wzroku, u których korekcja okularowa lub soczewkami kontaktowymi jest niewystarczająca, rozważa zabieg chirurgii refrakcyjnej. Dla pacjentów niekwalifikujących się do korekcji laserowej alternatywą są wszczepy fakijne. Cel pracy. Celem pracy jest ocena bezpieczeństwa stosowania collamerowych wszczepów fakijnych w korekcji wad wzroku podczas rocznej obserwacji. Materiał i metody. Zbadano 24 oczu 12 pacjentów, którym w celu korekcji krótkowzroczności wszczepiono soczewki fakijne tylnokomorowe Visian ICL. Przed zabiegiem wykonano refraktometrię obiektywną, ocenę ostrości do dali z najlepszą korekcją, pomiar ciśnienia wewnątrzgałkowego, ocenę śródbłonka, topografię rogówki oraz pomiar głębokości komory przedniej. Końcowe badanie obejmowało ostrość widzenia, refrakcję, ciśnienie wewnątrzgałkowe, położenie wszczepu, mikroskopię śródbłonka oraz badanie dna oka. Wyniki. Średnia refrakcja obiektywna przed operacją wynosiła: sfera −7,77 ±3,55 dioptrii, a cylinder 1,18 ±0,87 dioptrii. Korekcją okularową uzyskiwano średnią ostrość 0,91 ±0,20 (tablica Snellena). W okresie rocznej obserwacji średnia moc sfery wyniosła 0,475 ±0,390 dioptrii, cylinder 0,46 ±0,27 dioptrii, a ostrość widzenia po zabiegu 1,05 ±0,22 (tablica Snellena). Porównanie wyników w ocenie 12-miesięcznej wykazało istotnie statystycznie większą poprawę po zastosowaniu soczewki fakijnej w stosunku do korekcji okularowej (p < 0,05). Nie wykazano istotnych różnic w gęstości komórek śródbłonka w ocenie przed zabiegiem i po nim (p < 0,05). Zarówno w trakcie leczenia, jak i całego okresu obserwacji nie stwierdzono powikłań pooperacyjnych. Wnioski. Ocena bezpieczeństwa prawidłowo wszczepionych soczewek fakijnych ICL wskazuje na dobrą tolerancję implantu collamerowego w komorze tylnej. Implant nie wykazuje negatywnego wpływu na struktury przedniego odcinka, ze szczególnym uwzględnieniem śródbłonka rogówki i soczewki. Ostrość wzroku skorygowana metodą soczewki ICL nie ustępuje ostrości skorygowanej okularami, a w wielu przypadkach może być lepsza. Collamerowe wszczepy fakijne ICL mogą być stosowane jako alternatywa dla soczewek kontaktowych oraz refrakcyjnej wymiany przeziernej soczewki.
