Paget's Disease of the Vulva Treated with Imiquimod: Case Report and Systematic Review of the Literature

Abstract: Background: Extramammary Paget's disease of the vulva (EPDV) is a rare adenocarcinoma in situ of the vulvar skin and is often resected with involved margins due to its reticular growth pattern. Adjuvant treatment with the immunomodulator imiquimod may be suitable to avoid repeated and mutilating surgery. Case Presentation: We present the case of a 73-year-old woman with EPDV, initially treated with surgical resection and re-resection for involved margins. Final histology revealed Paget's disease of the left vu… Show more

“…Dogan et al 28 reported a pooled analysis where registered a CR of 71%, and the therapy was generally well tolerated with mild to moderate local reactions in more than 50% of cases. Photodynamic therapy, which uses a topical or intravenous photosensitizing agent that is activated by a light source to ablate abnormal tissue, offers a minimally invasive alternative.…”

“…11 A 2015 systematic review demonstrates high complete response rates (73%) with imiquimod therapy. 12,13 Mohs microsurgery (MMS) may provide an alternative surgical option with better assessment of surgical margins. Radiotherapy, topical chemotherapy with fluorouracil and bleomycin, photodynamic therapy, and laser therapy have also been proposed 11 ; however, data supporting these practices are limited by small case series.…”

“…3 In view of these data and considering that, in extensive disease, wide surgical excision carries the risk of a permanent functional impairment with significant impact in patient's quality of life, nonsurgical topical treatments have been tested. [5][6][7][8] Topical 5% imiquimod cream and PDT were the topical modalities mostly used, providing in many cases promising results. 5,6 There have been approximately 70 case reports in the literature, reporting satisfactory outcomes of vulvar EMPD with 5% imiquimod cream.…”
