Palmar Eccrine Hidradenitis Secondary to Trauma from Computer Gaming in an Adolescent After Bone Marrow Transplantation

Abstract: A 14-year-old boy who had undergone a matched sibling bone marrow transplant for acute lymphoblastic leukemia presented with painful nodules on his palms after prolonged gaming on his computer and mobile phone. Histology showed a neutrophilic inflammatory infiltrate surrounding the acrosyringium and eccrine sweat coils in the deep dermis. The lesions resolved spontaneously with conservative management.

“…Mechanical stresses induced by skin‐to‐skin rubbing, such as tension, strain and friction, exacerbate HS . Anecdotal case reports describe HS lesions developing in unusual body areas after mechanical stress and trauma . Despite these reports, the underlying mechanism(s) for why mechanical stress exacerbates HS symptoms are not entirely clear.…”

“…[7] Anecdotal case reports describe HS lesions developing in unusual body areas after mechanical stress and trauma. [7][8][9] Despite these reports, the underlying mechanism(s) for why mechanical stress exacerbates HS symptoms are not entirely clear.Skin responds to and resists mechanical stress through its cell-cell adhesions. Cell-cell adhesions are critical to maintain the structural integrity, tensile strength and barrier function of skin.Specifically, both adherens junctions (AJs) and desmosomes are mechanosensitive junctions responsible for sensing and reacting to mechanical forces by both altering the cytoskeleton and modulating downstream signalling cascades.…”

“…The excessive gaming in which our patients were engaged, many times that of the average American teen, has many potential consequences, of them being physical injury of the hands. Other reported injuries related to excessive gaming include de Quervain's tenosynovitis of the thumb (“gamer's thumb” or “nintendonitis”), punctate hemorrhage of the fingertips (“Playstation fingertip”), and rarely palmar eccrine hidradenitis, and are associated with excessive and strenuous use of gaming controllers, joysticks, cell phones, and other handheld devices 15‐19 . While there are many individuals who engage in computer gaming without developing pachydermodactyly or other disorders, excessive gaming may be a risk factor for pachydermodactyly in otherwise susceptible individuals.…”

“…Histology evidenced a focal neutrophilic inflammatory infiltrate surrounding the acrosyringium and eccrine sweat gland coils, consistent with palmar eccrine hidradenitis secondary to prolonged low-grade trauma. In this case too, cessation of video game playing led to spontaneous resolution of the lesions within two weeks [ 7 ].…”
