Pancreatic cavernous hemangioma: CT, MRI, US, and angiography characteristics

Abstract: A large pancreatic cavernous hemangioma was found in a 30-year-old man with abdominal distention. Plain and contrast-enhanced computed tomography (CT), magnetic resonance imaging (MRI), ultrasonography (US), and angiography were performed prior to operation. Contrast-enhanced CT and angiography showed a large poorly enhanced hypovascular tumor at the head of the pancreas. But MRI and US disclosed findings compatible with a cavernous hemangioma.

“…5 Of the 6 cases of pancreatic hemangioma with contrast-enhanced CT imaging to date, 5 cases (including ours) demonstrated this absence. 5,[7][8][9] EUS with FNA has been described as the most accurate modality for the diagnosis of pancreatic cysts. In our case, the EUS morphology was indistinguishable from that of a mucinous cystic neoplasm, whereas FNA cyst fluid analysis was nonspecific.…”

“…Herein, we report a case of adult pancreatic hemangioma. The mass showed a multilocular cyst with fluid-fluid levels and no obvious enhancement on CT and magnetic resonance imaging (MRI) whereas previously reported cases showed intense enhancement on CT after injection of contrast material, and high signal intensity on T2WI on MR (1-6). …”

“…Pancreatic hemangioma is very rare, especially in adults. Only a few cases of adult pancreatic hemangiomas have been reported in the English literature (1-6). They typically appear on computed tomography (CT) as a cystic lesion with contrast enhancement in the arterial phase.…”

“…In the case of large hemangioma complicated with Kasabach-Merritt syndrome which may be diagnosed by CT, MRI and/or angiography [7], conservative treatments including corticosterids, a-interferon [8], radiation therapy and combination chemotherapy with vincristine, actinomysin D and cyclophosphamide [3] are reported to be effective in involution of the tumor. Therefore, total primary excision should be abandoned.…”
