Pandemic Influenza A/H1N1 Virus Infection in Solid Organ Transplant Recipients: A Multicenter Study

Abstract: Mortality is higher in Tx recipients than in the general population. Poor outcome seems to be related to a delay in the beginning of treatment.

“…16,17 Studies investigating the role of pH1N1/ 09 infection in transplant patients have been limited to retrospective and observational case series. 7,18,19 The range of symptoms is comparable to the symptoms of seasonal influenza (from a self-limiting upper respiratory disease to respiratory failure). Morbidity and mortality rates in this group of patients are substantial, and they have high rates of hospitalization and admission to intensive care.…”

“…Morbidity and mortality rates in this group of patients are substantial, and they have high rates of hospitalization and admission to intensive care. 7,17,[19][20][21] Infectionassociated episodes of cellular rejection have been described. 2,8,22 Therefore, vaccination against pH1N1/09 appears to be essential and is strongly recommended as part of the pretransplant workup and posttransplant care.…”

“…In kidney transplant recipients, H1N1 caused significant morbidity and mortality [95][96][97][98], and mortality is higher in transplant recipients compared to the general population [97]. More severe cases develop pneumonia and may require ICU admission and ventilator support.…”

“…As seen in Table 3, a significant number of cancers are related to oncogenic viral infections. The Transplant Cancer Match Study assessed cancer risk in more than 175,000 solid organ transplant recipients, as compared to the general population [97]. It is important to note that this analysis includes only patients transplanted in the U.S., and the importance of biliary tract and bladder cancers due to parasitic infection outside of the U.S. are not represented in the analysis.…”
