Granuloma annulare (GA) is a benign, self-limiting granulomatous inflammatory disease that exhibits different histologic patterns. Infrequently, granuloma annulare can be associated with malignancy, the so-called malignancy-associated granuloma annulare (MGA). In this study, we aimed to compare the clinical and histopathological differences between GA and MGA. We retrospectively reviewed patient charts and identified 35 patients diagnosed with GA and concurrent hematological or solid organ malignancies as a case group. Additionally, we identified 33 patients without any known solid organ or hematological malignancy as a control group. MGA is commonly seen in the seventh decade of life, while GA affects the younger population. MGA is most commonly presented in the extremities of the body. The most common malignancy associated with MGA was chronic lymphocytic leukemia. Prostate cancer was the most common type of solid organ malignancy that was associated with MGA. The most common histopathological pattern seen in MGA was interstitial, comprising half of the cases. Multinucleated giant cells were present in half of the MGA cases and in most of the control group. In the literature, there are no established features that distinguish MGA from GA. Although MGA and GA have overlapping features, in our series, we found that the interstitial pattern was more common in MGA, while the necrobiotic pattern was more common in GA.

Introduction. Granuloma annulare belongs to the category of dermatoses characterized by necrobiosis in histopathological examination. The disease concerns mainly people under 30 years old. There are different types of granuloma annulare: localized, generalized and subcutaneous. According to the latest scientific reports, antimalarial drugs take a significant place in the treatment of generalized granuloma annulare. Objective. To present a case of a patient with generalized granuloma annulare and concomitant clinical signs of necrobiosis lipoidica who was treated with chloroquine with substantial improvement. Case report. A 54-years-old patient hospitalized at the Dermatology Department because of annular and irregular erythematous-atrophic lesions on upper and lower limbs. Ulcerations were present within the lesions on lower extremities. Histopathology was indicative of granuloma annulare. Treatment with chloroquine resulted in marked improvement. Conclusions. Generalized types of granuloma annulare are rarely observed and constitute a therapeutic challenge. Antimalarial drugs may be considered as the first-line treatment in generalized granuloma annulare. sTrEsZcZENiE Wprowadzenie. Ziarniniak obrączkowaty należy do dermatoz charakteryzujących się nekrobiozą w badaniu histopatologicznym. Choroba dotyczy zazwyczaj osób poniżej 30. roku życia. Wyróżnia się odmiany: ograniczoną, uogólnioną i podskórną. Według nowych doniesień naukowych ważne miejsce w leczeniu uogólnionego ziarniniaka obrączkowatego zajmują leki antymalaryczne. Cel pracy. Przedstawienie przypadku pacjentki z uogólnionym ziarniniakiem obrączkowatym i ze zmianami na kończynach dolnych klinicznie wykazującymi podobieństwo do obumierania tłuszczowatego. Opis przypadku. Kobieta 54-letnia hospitalizowana na oddziale dermatologii z powodu obrączkowatych i nieregularnych zmian rumieniowo-zanikowych na kończynach górnych i dolnych. BACKGROUND Paraneoplastic syndromes are characterized by atypical clinical manifestations. Several reports of hepatocellular carcinoma (HCC) paraneoplastic phenomena have been reported. They usually manifest as one type in an individual, but it is not common for the two clinical manifestations to occur simultaneously. CASE SUMMARY A 52-year-old female patient was admitted to hospital with pale skin and numbness of the second to fifth fingers in the left hand, which rapidly developed into severe digital ischemia. Computed tomography angiography revealed uneven thickness of the left ulnar artery with severe local luminal stenosis. Blood analysis during hospitalization showed persistent mild to medium thrombocytopenia and insensitive to hormonal therapy. Antiphospholipid antibody testing showed high titer of IgG anticardiolipin antibodies (aCLs), IgA aCLs, IgG anti-β2-glycoprotein-I (anti-β2 GPI), and IgA anti-β2 GPI. The exact diagnosis was HCC when the high a-fetoprotein levels, computed tomography findings, and the history of chronic hepatitis B came together. This was a rare case of coexisting manifestations as presenting symptoms of malignancy-associated antiphospholipid syndrome. The patient underwent several operations, antithrombotic treatments and hormonal therapy. However, the patient refused chemotherapy and died 8 wk after diagnosis. CONCLUSION This report highlights the importance of atypical clinical changes that could alert the physicians to vigilance for a concomitant underlying malignancy.
