Parastomal hernia repair: Bielañski hospital experience

Abstract: The most common occurred long-term stoma complication is parastomal hernia (PH). The incidence of this complication reaches 50% and, according to Goligher 1 , the parastomal hernia is an inevitable consequence in a certain percentage of all cases of stoma formation. The factors that may affect the incidence of parastomal hernia include the site of stoma, particularly its position relative to the rectus muscle of abdomen, preoperative mapping out of the stoma site, stoma diameter, intraperitoneal or extraperito… Show more

“…Local factors that influence the incidence of complications include: stoma topography, involvement of rectus abdominis muscle, preoperative demarcation, stoma diameter, transposition on intra-or extraperitoneal plane with fascia fixation, aponeurotic closure, emergency surgery and stoma type 6 , as well as higher predominance associated with females and obesity 7 .…”

“…These types describe different degrees of pathology of the stoma: from proper hernias (with presence of the hernial sac and its content) to conditions involving hernia recurrencesboth subcutaneous or through the stoma itself. Only the classification described by Szczepkowski includes in the assessed parameters coexistence and size of hernias in the postoperative scar after a vertical incision (1).…”

“…2 Goligher, even, went so far as to claim that some degree of parastomal herniation is inevitable given enough follow-up time. 3 Parastomal hernia is broadly defined as an incisional hernia located at or immediately adjacent to a stoma. 4 Due to the frequency of PSHs and the limited success rate of repair, attention has been focused on preventing PSHs at the outset when a stoma is fashioned.…”
