Parathyroid Autotransplantation During Thyroidectomy

Abstract: Although preservation of parathyroid glands in situ is desirable, routine parathyroid autotransplantation during thyroidectomy virtually eliminates postoperative hypoparathyroidism. Normal parathyroid glands resected or devascularized during thyroidectomy for well-differentiated thyroid carcinoma or benign disease should be transplanted in the sternocleidomastoid muscle. Patients with Multiple Endocrine Neoplasia type 2A should have parathyroid glands resected at the time of thyroidectomy for medullary thyroid… Show more

“…Although in most patients hypocalcemia recovers spontaneously, permanent hypocalcemia may be seen when the parathyroid glands are permanently damaged (8)(9)(10). In untreated patients with permanent hypoparathyroidism, serious complications such as cataracts, calcification of the basal ganglia and cerebellum, and papilla edema might develop (11-13).…”

“…Bilateral central (compartment VI) node dissection may improve survival (compared to historic controls) and reduce the risk for nodal recurrence (76,103). This central compartment dissection can be achieved with low morbidity in experienced hands (104)(105)(106)(107).…”

“…It has been generally accepted that half of one normal parathyroid gland can produce sufficient parathyroid hormone (PTH)2, 3. To avoid postoperative hypocalcaemia, parathyroid autotransplantation can be performed, although the results have been controversial4, 5.…”
