Background: Gynecomastia is a common benign lesion of the male breast and accounts for up to 80% of male patients referred with beast lesions. Objective: To examine the pattern of breast lesions in male patients who underwent breast biopsy. Methods: All male patients referred to Hatyai Hospital with breast complaints were included in the study. Histopathology reports were retrospectively reviewed during 2004-2011. Results: In 77 male patients diagnosed with breast lesions, the most common lesion was gynecomastia (49/77, 64%), followed by fibrocystic changes (7/77, 9%), and breast cancer (6/77, 8%). Gynecomastia was commonly found in male patients less than 40 years of age, while other breast lesions are seen in male patients over 40 years of age (P = 0.036, OR = 2.842, 95%CI = 1.085-7.447). Conclusion: Gynecomastia was the most common diagnosis among breast lesions in male patients referred to the Hatyai Hospital during 2004-2011. Gynecomastia was more common in the patients less than 20 years old.

ÖzetAmaç: Erkeklerde meme kanseri nadir rastlanan tümörlerdendir. Bu çalışmada meme polikliniğimize başvuran hastalardan meme kanseri tespit edilen erkek hastalar incelenmiştir. Yöntem: Haseki Eğitim ve Araştırma Hastanesi Genel Cerrahi Kliniği'nde meme kanseri tanısı ile opere edilen altı erkek hasta geriye dönük olarak incelenmiştir. Hastalar, Ocak 2009-Aralık 2012 tarihleri arasında meme polikliniğine başvurup meme kanseri tanısı alan 268 hasta arasından seçilmiştir. Bulgular: Hastaların yaş ortalaması 61 yıl (50-73) idi. En sık başvuru nedeni memede ele gelen kitleydi. Tümör çapı ortalaması 2.58 cm (2-3.5) idi. Olguların tümüne modifiye radikal mastektomi uygulandı. Tüm hastalarda preoperatif kor biyopside olduğu gibi postoperatif histopatolojik olarak invazif duktal karsinom tanısı doğrulandı. Preoperatif yapılan görüntülemelerde ve postoperatif patolojik incelemelerde tüm hastalarda aksiller lenf nodu tutulumu saptandı. Hastalara uygun adjuvan tedaviler uygulandı. Sonuç:Erkek meme kanseri ile kadın meme kanseri aynı evrede ise benzer prognoza sahiptir. In this study, we aimed to evaluate the male patients who were diagnosed with breast carcinoma and admitted to our outpatient clinic. Methods: Among 268 patients diagnosed with breast carcinoma in the General Surgery Department at Haseki Training and Research Hospital between January 2009 and December 2012, a total of six male patients, who were operated on for breast cancer, were retrospectively reviewed. Results:The mean age of the subjects was 61 years (range: 50-73). The most clinical presentation was a palpable breast mass. The mean diameter of the lesions was 2.58 cm (range: 2.0-3.5 cm). All patients underwent modified radical mastectomy. The postoperative histopathology confirmed the diagnosis of invasive ductal carcinoma consistent with the results of preoperative core biopsies. Preoperative imaging studies and postoperative pathological analyses indicated axillary lymph node positivity. Postoperative adjuvant therapies were performed. Conclusion: The prognosis of breast cancer is similar in male and female patients when detected in similar stages. Although breast cancer is very rare in men, it is detected in advanced stage because of late diagnosis. Over the past 2 decades, the percentage of men presenting with breast complaints has increased from 0.8% to 2.4%, and men now account for 1% of all breast cancer cases. The most common male breast mass is gynecomastia, followed by lipoma and epidermal inclusion cysts. Because there is a paucity of parenchyma as compared with the female breast, the malignancy rapidly progresses to the next stage, with the appearance of secondary signs like nipple retraction, fixation to deeper tissues, skin ulceration or adenopathy. Diagnostic evaluation is needed only when the palpable mass is unilateral, hard, fixed, peripheral to the nipple, or associated with nipple discharge, skin changes, or lymphadenopathy. Male breast cancer usually occurs in a subareolar location or is positioned eccentric to the nipple; occasionally, it occurs in a peripheral position. Secondary signs like skin thickening, nipple retraction, and axillary lymphadenopathy may be seen. Microcalcifications can occur. Mammography can accurately distinguish between malignant and benign male breast disease. Radiologists are generally less familiar with breast disease in males compared with females. In this article, we discuss the clinical, and mammographic features of a variety of benign and malignant diseases that can occur in the male breast.
