Patients With Vestibular Migraine are More Likely to Have Occipital Headaches than those With Migraine Without Vestibular Symptoms

Abstract: Objective To determine whether patients with vestibular migraine are more likely to suffer from an occipital headache than patients with migraine without vestibular symptoms. Background Vestibular migraine is an underdiagnosed disorder in which migraine is associated with vestibular symptoms. Anatomical evidence and symptomatology hint at the involvement of brain structures in the posterior fossa (back of the head location). We hypothesized that vestibular migraine patients are more likely than migraineurs wit… Show more

“…(ICHD-3, 2018 ). Approximately 10%–30% of patients in headache and dizziness clinics are diagnosed with vestibular migraine, with the condition affecting about 1%–3% of the total population (Neuhauser et al, 2006 ; Formeister et al, 2018 ; Wattiez et al, 2020 ) and accounting for 10% among migraine patients (ICHD-3, 2018 ). The current treatments for acute vestibular migraine attacks include triptans (which are effective in 40% of patients) and antiemetic medications (Shen et al, 2020 ).…”

“…How these changes in the cerebellar microenvironment may lead to headache is unknown, but anatomical and physiological data from both humans and animals suggest a possible connection of events in the posterior fossa, with activation of afferent sensory information from the posterior dura to the brainstem trigeminocervical complex via C2/3 DRG cervical innervation ( 4 , 350 , 351 ). Lastly, given that pain in occipital areas of head, including neck muscle tenderness, is experienced by a large number of migraineurs, and that patients with vestibular migraine appear more likely to experience headaches in the occipital region of the head ( 352 ), it is possible that the cerebellum is playing a larger role in migraine pathophysiology than previously thought.…”
