Background Rheumatoid arthritis (RA) is a chronic inflammatory disease affecting about 0.5–2% of the population worldwide. Extra-articular involvement in RA is a common condition and can occur in up to 40% of patients. Pulmonary involvement is a common extra-articular manifestation and may affect parenchyma, pleura, airway, and vasculature. It occurs in 20 to 67% of RA patients and it is responsible for about 10–20% of mortality in RA patients. So evaluation of the patients with rheumatoid arthritis for any deterioration in their lung functions is essential to treat appropriately, improve their mobility and quality of life. Objectives The study aimed to evaluate the effect of RA on the pulmonary function tests (PFTs) among Sudanese RA patients attending Rheumatology clinics in Omdurman Military hospital. Methods This is an analytical cross-sectional hospital -based study conducted among Sudanese RA patients. The study included 32 RA patients who were not known to have any respiratory illnesses or chronic diseases. An interview questionnaire with personal data and brief medical history was filled by the investigator. Height and weight were measured and BMI was calculated. Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 second (FEV1), and Peak Expiratory flow Rate (PEFR) were measure for each participant by digital spirometer. Matching from control group with similar age and height was performed and the results were compared with normal reference values for Sudanese subjects.. The tested data was analyzed using SPSS version 25, t-test was used to compare the statistical significant (P-value ≤ 0.05 was considered statistical significant). Results In this study, all the measured parameters of the PFTs in RA patients were lower than the published reference normal values for Sudanese subjects with significant P values. The mean of FEV1, FVC, FEV1/FVC ratio and PRFR in RA patients was found to be 1.8 ± ,3 L/min, 2,1 ± ,4 L/min, 87.1 ± 12,2% and 223.6 ± 86.2 L/min respectively in compared to 2,4 ± ,1 L/min, 2,7 ± ,1 L/min,90.81 ± 2.6% and 345.4 ± 86.2 L/min respectively in the control group (P Value 0.001, 0.001,0.05, and 0.001 respectively). There was no statistical significance correlation between PFT and duration of age, disease, and BMI. Obstructive ventilatory defect was seen in 78.1% (25) of patients. Restrictive ventilatory defect was seen in only 6.3% (2) of patients and 15.6% (5) showed normal pattern. Conclusion Pulmonary function abnormalities are common in asymptomatic rheumatoid patients which were found to be obstructive defect. Different pattern of pulmonary function abnormalities had a manifested in RA patients and the obstructive pattern represents the most common feature. Spirometry is indicated as a baseline assessment and for follow up of RA patient to enhance early detection and management of the pulmonary involvement.

Background As a common extra-articular symptom of rheumatoid arthritis (RA), pulmonary involvement affects 20–67% of patients and accounts for 10–20% of RA-related deaths. This study aimed to assess the impact of RA on pulmonary function tests (PFTs) in Sudanese RA patients attending the rheumatology clinics of the Omdurman Military Hospital. Methods This was an analytical, cross-sectional, hospital-based study of 32 RA patients who met the 2010 Rheumatoid Arthritis Classification Criteria of the American College of Rheumatology (RA ACR/EULAR) and who were nonsmokers and free of known respiratory or chronic diseases. Pulmonary function parameters (PFTs), including forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), the FEV1/FVC ratio, and the peak expiratory flow rate (PEFR), were measured for each participant using a digital spirometer. A control group of Sudanese subjects of similar age, sex, and height was matched, and the results were compared with normal reference values. Results A high prevalence of lung function abnormalities (93.8%) was observed in RA patients, and 6.2% of them had normal PFT parameters. Restrictive lung disease was recorded in 78.1% of the patients, and obstructive lung disease was observed in 15.7% of the patients. All measured PFT parameters in the RA patients were significantly lower than the published reference normal values for Sudanese subjects, except for the FEV1/FVC ratio. The mean values of FEV1, FVC, the FEV1/FVC ratio, and the PRFR in RA patients were 1.8 ± 0.3 L/min, 2.1 ± 0.4 L/min, 87.1 ± 12.2%, and 223.6 ± 86.2 L/min, respectively, compared to 2.4 ± 0.1 L/min, 2.7 ± 0.1 L/min, 90.81 ± 2.6%, and 345.4 ± 16.2 L/min, respectively, in the control group (P values of 0.001, 0.001, 0.299, and 0.001, respectively). There was no statistically significant correlation between PFT results and disease duration, age, or BMI. Conclusions Regardless of the duration of RA, restrictive lung abnormalities are frequent in asymptomatic RA patients. The PFT can serve as an indicator of RA. Spirometry has been advised as a baseline examination and for follow-up and to promote the early detection and management of pulmonary involvement in RA patients.

Background and Objective: Pulmonary involvement is a common extra-articular manifestation of rheumatoid arthritis (RA). The present study attempts to highlight the extent of involvement by assessing lung function using pulmonary function tests (PFTs) in patients with RA. Methods: This pilot study involved 30 patients with RA and an equal number of age- and sex-- matched healthy controls. All the study participants were subjected to spirometry and in RA patients, the test was repeated after 30 minutes of administering 400 mcg of salbutamol. PFT values of the controls and the pre and post-values of cases like forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), Percentage of forced expiratory volume in the first second (FEV1%), forced expiratory flow at 25% to 75% of the lung volume (FEF25-75%) and peak expiratory flow rate (PEFR) were recorded. Independent sample t-test or Mann-Whitney U test were used for comparisons. The pre- and post-values of PFT were compared using paired t-test, Welsch’s test, or Wilcoxon tests. (p ≤ 0.05 implied statistical significance). Results: PFT variables were significantly lower among patients with RA compared with controls (p<0.05). A significant negative correlation of FEF25%-75% and PEFR with BMI in obese patients (BMI >23) and FVC, FEV1, and PEFR significantly lower in post-menopausal women (p < 0.05) was seen. Conclusion: Based on the pilot study findings, patients in south India with RA could manifest decreased pulmonary functions. Obese patients and post-menopausal females with RA could be at a higher risk, and the administration of nebulized salbutamol could improve pulmonary functions.
