Patterns of Regional Spread for Esthesioneuroblastoma

Abstract: BACKGROUND AND PURPOSE: ENB is a rare malignant neoplasm that affects the anterior skull base. Disease stage is a significant predictor of survival, in particular the presence or absence of lymph node metastases. Multiple studies have identified the cervical lymph nodes as the most frequent site of spread; however, no studies have attempted to characterize the radiographic appearance of metastatic lymph nodes or identify the primary nodal drainage for these tumors.

“…Adjuvant chemotherapy was performed with cisplatin and etopside, but without the subsequent proton radiaton used by Harvard University in a series of 9 patients, with good results (24). Although described as rare, retropharyngeal lymph node metastasis was considered as frequent in a study by Howell et al (25). Despite the absence of a central nervous system invasion, the patient in the present study showed a vertebrae lesion with somatostatin analogue uptake, highly suggestive of secondary involvement.…”

“…Although the standard PET/CT protocol at our institution uses low-dose, unenhanced CT, the addition of diagnostic CT with intravenous contrast material would likely result in detection of additional lesions and improved PET/CT performance, especially given that both primary esthesioneuroblastoma tumors and metastases show avid contrast enhancement (8)(9)(10). The role of PET/CT performed with full-dose contrastenhanced CT in esthesioneuroblastoma patients should be evaluated, including its performance compared with and in conjunction with MRI.…”

“…18 F-FDG PET/CT has proven to be a useful adjunct to conventional imaging in more common malignancies, leading to improved pretreatment staging, assessment of treatment response, and differentiation of early recurrence from therapy-induced changes. To date, MRI and CT have been the primary imaging modalities used for evaluation of patients with esthesioneuroblastoma (8)(9)(10)(11). Limited research has been performed on the role of 18 F-FDG PET/CT in esthesioneuroblastoma (12); the largest series to date included 12 patients having undergone PET/CT, and the modality was deemed appropriate for detecting cervical metastasis given their propensity for 18 F-FDG avidity (8).…”

“…Although this tumour may occur in patients of any age, the literature points to a bimodal distribution, with peaks in the 2nd and 3rd decades and the 6th and 7th decades of life. 4 The Kadish staging system is applied to these tumours. 5 Stage A: disease confined to the nasal cavity Stage B: disease confined to the nasal cavity and paranasal sinuses Stage C: local or distant spread beyond the nasal cavity or sinuses The differential diagnosis of ENB includes the group of small round blue cell malignant neoplasms that can occur in the sinonasal tract, i.e.…”
