Pearls and paradigms in Infective Keratitis

Abstract: Infective Keratitis is a commonly encountered sight-threatening ocular emergency. In developing countries, it is a major cause of corneal blindness. Empirical treatment without microbiological work up often leads to treatment failure. Indiscriminate use of steroid antibiotic combination worsens the situation and makes further management challenging. The correct line of management can be potentially sight saving for both the ophthalmologist and the patient. This article on Infective keratitis has been written k… Show more

“…The method of obtaining the corneal scrapes has been described with many instruments, such as sterile cotton swab, sterile calcium alginate swab, heatsterilized platinum (Kimura) spatula, 26 or 23-gauge needle, blade, cotton-tipped applicator 2,33,34 . In our study, we used sterile steel spatulas that were changed for each smear.…”

“…Since the bacterial ophthalmic infections (both corneal, keratitis, and intraocular, endophthalmitis) are still one very important cause of eye disease, unquestionably, it is critical to improve the clinical results with antimicrobial agents [33]. Further studies on this topic are warranted.…”

“…The presence or absence of hypopyon was recorded and the height measured in millimeters. Based on the severity grading system for infectious corneal ulcers, 8 , 9 patients with AK were graded as mild (corneal ulcer diameter <2 mm, ulcer depth <20%, and infiltration depth <20%), moderate (corneal ulcer diameter 2–5 mm and/or ulcer depth 20%–50% and/or infiltration depth 20%–50%), and severe (corneal ulcer diameter >5 mm or ulcer depth >50% or infiltration depth >50%). The grouped results were graded with priority given to the most serious criteria.…”

“…Similarly, as persistent inflammatory responses are present in some cases of AK, systemic or local steroids, or immunosuppressive agents, have occasionally been used, but no clear consensus has been defined on their use. 7 – 9 Consequently, it is important to investigate the mechanisms of inflammation in AK as well as the relationship between Acanthamoeba and the corneal inflammatory response.…”
