Pedicle screw versus hybrid instrumentation in adolescent idiopathic scoliosis

Abstract: Background:Incidence of complications and reoperations between pedicle screw (PS) and hybrid instrumentations (HI) are still controversial in adolescent idiopathic scoliosis (AIS) patients. A systematic review and meta-analysis were performed to compare overall complications, reoperations, and radiographic outcomes between the 2 constructs. Methods:Strictly followed the PRISMA 2009 guidelines, the MEDLINE, EMBASE, and the Cochrane Library databases were used to search for literatures up to April 2016, addressin… Show more

“…Like with RCT, only 4 meta-analyzes were able to demonstrate statistically significant results (Table 5): Vancomycin powder reduces the rate of spinal implant infection, sacral 2 alar iliac screw fixation showed a lower risk of infection compared to iliac screw fixation, and pedicle screws demonstrated a significantly higher rate of reoperations for reinfection than hybrid instrumentation. 57,58,76,77 In contrast significantly reduced risk for infection was not identified with extended postoperative and preoperative antimicrobial prophylaxis, or different screw fixation techniques. 74,78,79,80-84…”

“…The pedicle screw system has advantages of strong pull-out strength resistance, multi-plane stability, less fusion joints, less postoperative support 3 . When applied to correction surgery, it has strong correction strength, lower incidence of complication and lower reoperation rate 4,5 . Nowadays, freehand technique is the most common technique of pedicle screw placement.…”

“…For the severe scoliosis, correction surgery is essential to reconstruct the alignment. [ 3 ] It is commonly accepted that Cobb angle is frequently used to quantify the severity of this deformity. [ 4 ] In addition, the Cobb angle based on anteroposterior radiograph is the necessary prerequisite to plan the surgery, and correction surgery is always required when a Cobb angle > 50°.…”
