Peginterferon alpha and ribavirin combination therapy in patients with hepatitis C virus-related liver cirrhosis

Abstract: Background/AimsPegylated interferon (peginterferon) and ribavirin combination therapy is less effective and associated with a higher frequency of serious complications in chronic hepatitis C patients with cirrhosis than in noncirrhotic patients. This study evaluated the efficacy and tolerability of peginterferon and ribavirin treatment in patients with hepatitis C virus (HCV)-related cirrhosis.MethodsEighty-six patients with clinically diagnosed liver cirrhosis were treated with either peginterferon alpha-2a (… Show more

“…Chen et al 24 reported SVR rate in the patients at the age of 19-76 with non-genotype 1 HCV infection was 91.4%, while SVR rate of HCV genotype 1 was 66.3%, higher viral load appeared to contribute more to the SVR. However, Kim et al 25 reported HCV genotype 1 and high baseline viral load (HCV RNA >8.0×10 5 IU/mL) were associated with low SVR rate in the patients at the age of 46 to 66. In addition, our results indicated that there was no correlation between SVR and the baseline HCV RNA viral load.…”

“…In a recent study of South Korean patients diagnosed with liver cirrhosis clinically that received peginferferon and ribavirin combination therapy, SVR rates were reported to be 20.8% for genotype 1 patients and 52.6% for genotype 2 or 3 patients 20. In the multivariate analysis, HCV genotype 1 is an independent factor associated with low SVR rates 20.…”

“…In the multivariate analysis, HCV genotype 1 is an independent factor associated with low SVR rates 20. It is generally well-known that chronic hepatitis C patients without cirrhosis have low SVR rates if they have HCV genotype 1 and a high viral load prior to treatment 20.…”

“…Therefore, there is an urgent need for predictive genetic tools such as those based on SNP profiling (referred as personalized medicine), to characterize patients with susceptibility to HCV infection and HCV-mediated liver diseases. This will allow for better predictions in HCV infection progression rates and improvements in the outcome of standard therapies involving pegylated IFN- α combined with ribavirin [24]. TLR3, a sensor of the host immune system activated by viral dsRNA and responsible for the production of IFN, has been demonstrated to play an essential role during HCV infection and clearance, as well as in the severity of liver diseases.…”
