Penetrating and Orthopaedic Trauma from Blast versus Gunshots Caused by Terrorism: Israel's National Experience

Abstract: Musculoskeletal injury caused by blast is associated with multiple penetrating injuries, differing from more "conventional" penetrating trauma such as caused by gunshot injuries. Although the treatment of specific injuries caused by both mechanisms is essentially similar, the systemic effect of the blast must be taken into consideration in anticipation of an increased overall injury severity.

“…Various studies examining the incidence of injuries following blast events have been conducted throughout the twentieth century in the context of domestic and international terrorism and warfare. Compiling these results provides a picture of the body areas where one can expect injuries to be located . Figure illustrates the statistics in these studies by demonstrating the percentage of victims with injuries to the five body regions in both civilian terrorism and combat situations.…”

“…The explosive blast overpressure wave triggers systemic polytraumatic injury across multiple body organs [3]. Compared to single-mechanism penetrating trauma, blast-injured patients exhibit higher injury severity scores, often leading to higher mortality, disabilities, and longer hospitalization [4]. Burns resulting from combat explosions in recent military operations have increased in frequency and severity score [5].…”

“…Thus, the frequency of these injuries can vary enormously. The Second Intifada or Palestinian political violence between 2000 and 2005 constituted the largest recent surge in injuries treated at our hospital, with hundreds of victims of shooting or blast injuries (Luria et al., 2013; Weil et al., 2011). Regretfully, political unrest is a constant problem in Israel.…”

“…Treatment is managed according to standard trauma principles (Bartlett, 2003; Coupland, 1989; Weil et al., 2011). Ischemia of the upper limb should be assessed on arrival, in addition to the extent of bone and nerve damage.…”
