Perception of self: distinguishing autoimmunity from autoinflammation

Abstract: Rheumatic diseases can be divided in two groups, autoinflammatory and autoimmune disorders. The clinical presentation of both types of diseases overlap, but the pathological pathways underlying rheumatic autoinflammation and autoimmunity are distinct and are the subject of ongoing research. There are a number of ways in which these groups of diseases differ in terms of disease mechanisms and therapeutic responses. First, autoinflammatory diseases are driven by endogenous danger signals, metabolic mediators and… Show more

“…As diverse autoantibodies (e.g. Recent classification of inflammatory diseases into autoimmune, mixed and auto-inflammatory diseases takes these differences into account (McGonagle & McDermott 2006;Kastner et al 2010;Abramovits & Oquendo 2013;Davila-Seijo et al 2014;van Kempen et al 2015;Pathak et al 2017). Self-directed inflammation by autoinflammatory diseases is caused by an overactivity of the adaptive and/or innate immune system, without specific identification of autoreactive B-and Tcell responses (e.g.…”

“…Its role in ICD depends on the TLR4 MyD88 signaling route and the subsequent release of interleukin (IL)‐1β by activated DC (Figure ) . However, neither PAMP nor DAMP are able to trigger adaptive immunity alone unless antigenic neo‐epitopes from either microbial or oncogenic origin, which have not provoked central or peripheral tolerance earlier, are present …”

“…Rheumatic disorders can be divided into primarily autoimmune and autoinflammatory, with some overlap, but each having distinct mechanisms [11]. Autoinflammatory disorders are primarily of the innate immune response and driven by inflammasome-induced IL(interleukin)-1b and IL-18 [12].…”
