Percutaneous Thrombin Injection of a Femoral Artery Pseudoaneurysm with Simultaneous Venous Balloon Occlusion of a Communicating Arteriovenous Fistula

Abstract: An 82-year-old woman developed acute occlusion of her right coronary artery. She underwent percutaneous coronary stent placement and aortic balloon pump installation. In the postprocedural period, she developed a common femoral artery pseudoaneurysm (PSA) that communicated with the common femoral vein via an arteriovenous fistula (AVF). After unsuccessful ultrasound-guided compression, ultrasound-guided thrombin injection of the PSA was performed, with simultaneous balloon occlusion of the common femoral vein … Show more

“…For instance, one case report described a successful percutaneous thrombin injection of a femoral artery pseudoaneurysm while simultaneously occluding the venous outflow of a communicating arteriovenous fistula with a balloon. 12 Surgical treatment, while being largely replaced by less invasive image-guided interventions, is still preferred in cases of rapidly enlarging pseudoaneurysms, especially in the presence of hemodynamic instability, distal ischemia or neuropathy, impending compartment syndrome, infection, or failed image-guided intervention. 2 Evacuation of hematoma followed by suturing or patch angioplasty of the damaged arterial wall is a common surgical approach for pseudoaneurysm repair.…”

“…Another method of treating FAP was percutaneous thrombin injection which was first described by Cope and Zeit [17]. Current practice typically involves percutaneous thrombin injection with ultrasound visualization of the needle tip within the FAP [18,19].…”

“…Geralmente, as fístulas arteriovenosas não necessitam de tratamento invasivo, podendo-se optar pelo tratamento conservador, uma vez que na grande maioria dos casos pode-se ter resolução espontânea [9][10][11] . Porém, naqueles pacientes em uso de anticoagulantes e antiplaquetários, a taxa de resolução espontânea é pequena.…”
