Performance of Commercial Enzyme-Linked Immunoassays for Diagnosis of Herpes Simplex Virus-1 and Herpes Simplex Virus-2 Infection in a Clinical Setting

Abstract: US Food and Drug Administration-approved EIAs have poor PPV for HSV-2 and poor NPV for HSV-1 in clinical practice. More accurate rapid type-specific HSV antibody tests are needed.

“…Lack of symptoms in a person who is HSV-1 seropositive does not distinguish anogenital from orolabial or cutaneous infection, and, regardless of site of infection, these persons remain at risk for acquiring HSV-2. In addition, HSV-1 serologic testing has low sensitivity for detection of HSV-1 antibody (458). However, acquisition of HSV-1 genital herpes is increasing, and HSV-1 genital herpes also can be asymptomatic (437)(438)(439)461,462).…”

“…The most commonly used test, HerpeSelect HSV-2 enzyme immunoassay (EIA), often is falsely positive at low index values (1.1–3.0) ( 457 457 ). One study reported an overall specificity of 57.4%, with a specificity of 39.8% for index values of 1.1–2.9 ( 458 ). Because of the poor specificity of commercially available type-specific EIAs, particularly with low index values (<3.0), a confirmatory test (Biokit or Western blot) with a second method should be performed before test interpretation.…”

“…Enzyme immunoassay (EIA) testing (that is, He rpeSelect HSV-2 EIA) should be followed by confi rmation testing because of poor specifi city and high false-positive rates in EIA tests. 1,26 Use of confi rmatory tests (BioKit or Western blot) increases accurate identifi cation of HSV-2. Clear diagnosis of HSV-2, which is almost always sexually acquired, provides opportunity for focused education to reduce spread.…”
