Perivascular epithelioid cell neoplasm (PEComa) of the uterus: A systematic review

Abstract: Although the optimal treatment of gynecological PEComas is controversial, surgical resection remains the cornerstone. The use of adjuvant treatment is warranted in high risk patients to increase disease control. A multidisciplinary approach should be key in treatment decision-making regarding gynecological PEComas.

“…Moreover, these cells are characterized immunoreactive for melanocytic markers, with an epithelioid appearance and a clearacidophilic cytoplasm, thus the term “perivascular epithelioid” was introduced [2] . Progressively, this family of tumors came to include angiomyolipoma, clear cell sugar tumors, lymphangioleiomyomatosis, clear cell myomelanocytic tumor of the falciform ligament and other clear cell tumors [3] . According to World Health Oraganization (WHO), PEComas are now defined as mesenchymal tumors composed of histologically and immunohistochemically distinctive perivascular epithelioid cells [4] .…”

“…Отдельные публикуемые случаи [15] не дают чёткого представления о преимуществах и недостатках такого комбинированного подхода по сравнению с только хирургическим. При этом ряд авторов полагает оправданным проведение адъювантного лечения, в том числе лучевой терапии как одного из вариантов, у пациентов с определёнными клиническими ситуациями, например, у пациенток с ПЭКомой тела матки высокого риска [16].…”
