Pharmacokinetics of remifentanil and its major metabolite, remifentanil acid, in ICU patients with renal impairment †

Abstract: Although RA accumulates in patients with moderate/severe renal impairment, pharmacokinetic modelling predicts that RA concentrations during a 9 microg kg(-1) h(-1) remifentanil infusion for up to 15 days would not exceed those reported in the present study, for which no associated prolongation of mu-opioid effects was observed.

“…Finally, some indicators such as t 1 2 ,α = 1.85h, t 1 2 ,β = 12.2h, V ml = 0.626 4 were calculated in accordance with results in Ref. [26]. Pharmacokinetics of remifentanil acid for patients with severe degree of renal impairment will be studied in the future.…”

“…As a new analgesic used in Chinese hospitals, its pharmacokinetics in Chinese population is not yet thoroughly studied. One-, two-and three-compartment models have previously been used to the pharmacokinetics of remifentanil in humans [25,26] . A two-compartment model was used in patients with mild degrees of renal impairment without theoretical deduction.…”

“…In the elderly remifentanil clearance is reduced (Minto et al 1997b). This observation, associated with the well-known increase of elderly patients' susceptibility to the pharmacological action of opioids, should lead to markedly decreased dosing recommendations in this population (Kabbaj et al 2005;Pitsiu et al 2004;Bouillon et al 2002).…”

“…It is excreted through the kidney. Even when accumulation occurs in renal failure patients, the concentrations obtained after a few hours of infusion are insufficient to generate pharmacological effects (Hoke et al 1997;Pitsiu et al 2004). “…The context half-life of remifentanyl is 2 -3 min and is independent of the duration of infusion. Remifentanyl is eliminated via the kidneys, and its elimination is prolonged in patients with severe renal impairment [4,5]. The determination of analgesics -anaesthetics in biological fluids is of major clinical importance in order to accurately monitor their concentration levels and assist/stir therapeutic intervention.…”
