BackgroundKnee osteoarthritis (OA) is one of the leading causes of disability within the adult population. Current treatment options for OA of the knee include intra-articular (IA) hyaluronic acid (HA), a molecule found intrinsically within the knee joint that provides viscoelastic properties to the synovial fluid. A variety of mechanisms in which HA is thought to combat knee OA are reported in the current basic literature.MethodsWe conducted a comprehensive literature search to identify currently available primary non-clinical basic science articles focussing on the mechanism of action of IA-HA treatment. Included articles were assessed and categorized based on the mechanism of action described within them. The key findings and conclusions from each included article were obtained and analyzed in aggregate with studies of the same categorical assignment.ResultsChondroprotection was the most frequent mechanism reported within the included articles, followed by proteoglycan and glycosaminoglycan synthesis, anti-inflammatory, mechanical, subchondral, and analgesic actions. HA-cluster of differentiation 44 (CD44) receptor binding was the most frequently reported biological cause of the mechanisms presented. High molecular weight HA was seen to be superior to lower molecular weight HA products. HA derived through a biological fermentation process is also described as having favorable safety outcomes over avian-derived HA products.ConclusionsThe non-clinical basic science literature provides evidence for numerous mechanisms in which HA acts on joint structures and function. These actions provide support for the purported clinical benefit of IA-HA in OA of the knee. Future research should not only focus on the pain relief provided by IA-HA treatment, but the disease modification properties that this treatment modality possesses as well.

The aim of this study is to review developments in glycosaminoglycan and proteoglycan research relevant to cartilage repair biology and in particular the treatment of osteoarthritis (OA). Glycosaminoglycans decorate a diverse range of extracellular matrix and cell associated proteoglycans conveying structural organization and physico-chemical properties to tissues. They play key roles mediating cellular interactions with bioactive growth factors, cytokines, and morphogenetic proteins, and structural fibrillar collagens, cell interactive and extracellular matrix proteoglycans, and glycoproteins which define tissue function. Proteoglycan degradation detrimentally affects tissue functional properties. Therapeutic strategies have been developed to counter these degenerative changes. Neo-proteoglycans prepared from chondroitin sulfate or hyaluronan and hyaluronan or collagen-binding peptides emulate the interactive, water imbibing, weight bearing, and surface lubricative properties of native proteoglycans. Many neo-proteoglycans outperform native proteoglycans in terms of water imbibition, matrix stabilization, and resistance to proteolytic degradation. The biospecificity of recombinant proteoglycans however, provides precise attachment to native target molecules. Visco-supplements augmented with growth factors/therapeutic cells, hyaluronan, and lubricin (orthobiologicals) have the capacity to lubricate and protect cartilage, control inflammation, and promote cartilage repair and regeneration of early cartilage lesions and may represent a more effective therapeutic approach to the treatment of mild to moderate OA and deserve further study.Similar protective perineural net structures assembled from the lectican proteoglycan family and HA also occur in the CNS/PNS. These so called perineuronal nets are visualized by immunolocalization of MAb 1B5 (+) epitope in rat brain (e) and pericellular 1B5(+) epitope expression by isolated neurons (f). Scale bars 100 µm.

Background and Objectives: Osteoarthritis is an inflammatory and degenerative joint disease, causing pain, musculoskeletal disorders and would affect functionality, daily life activities and quality of life. Comparing the effects of ultrasound and microwave physiotherapy treatments on knee osteoarthritis by measuring osteocalcin level and erythrocyte sedimentation rate in blood. Methods: In a cross-sectional study conducted in in Hawler teaching hospital and Rzgari teaching hospital over the period December 2018 to May 2019, 30 patients with knee joint osteoarthritis were recruited. Half of them were provided with ultrasound physiotherapy treatment, and half with microwave physiotherapy treatment. The level of serum osteocalcin and erythrocyte sedimentation rate in blood were measured in both groups before and after the treatment with lasted for one month. The collected data were analyzed through paired and independent samples t-test using Statistical Package for the Social Sciences (version 18). Results: Most of the patients (73.3%) were females. Right knee osteoarthritis was the most prevalent with 13 patients, followed by bilateral knee osteoarthritis with 10 cases, and left knee osteoarthritis with 7 patients. The results indicated that ultrasound physiotherapy treatment had a significant effect on the level of serum osteocalcin (p=0.01) and erythrocyte sedimentation rate in blood (p=0.000). It was also seen that microwave physiotherapy treatment had a significant effect on erythrocyte sedimentation rate in blood (p=0.000), while its effect on the level of serum osteocalcin was not significant (p=0.53). Conclusion: Based on the data collected from the present study, there is sufficient evidence to indicate that ultrasound and microwave physiotherapy interventions can reduce pain and improve function in those with knee OA.

Abstract: A single-dose IA injection of Gel-200 exerts chondroprotective and anti-inflammatory effects in the experimental OA model, and long-lasting analgesia in the joint pain model, suggesting the beneficial multimodal function of Gel-200 against symptomatic OA patients.
