Pharmacology, Pathophysiology and Management of Calcium Channel Blocker and ??-Blocker Toxicity

Abstract: Calcium channel blockers (CCB) and beta-blockers (BB) account for approximately 40% of cardiovascular drug exposures reported to the American Association of Poison Centers. However, these drugs represent >65% of deaths from cardiovascular medications. Yet, caring for patients poisoned with these medications can be extremely difficult. Severely poisoned patients may have profound bradycardia and hypotension that is refractory to standard medications used for circulatory support.Calcium plays a pivotal role in c… Show more

“…2, mean MAP of each group decreased nearly identically. GLM evaluation of the data revealed an effect of time on MAP, but no effect of group (F (1,8) =0.11, p=0.75). GLM evaluation of heart rate also revealed no effect of group (F (1,8) =0.04, p=0.839).…”

Interestingly, rats in both groups maintained heart rates with little to no change until shortly before death at which point heart rates decreased precipitiously (Figs.…”

“…Due to pharmacological selectivity, dihydropyridine CCBs (e.g. amlodipine, nifedipine) primarily affect the calcium channels in the smooth muscle6 and, in cases of overdose, cause vasodilatory shock with reflex sinus tachycardia 7, 8. Phenylalkylamines such as verapamil and benzothiazepines such as diltiazem cause cardiogenic shock in the case of overdose.…”
